VENTRICULAR ARRHYTHMIAS IN NORMAL HEARTS - 05/09/11

Idiopathic ventricular tachycardia (VT) once was considered an undifferentiated diagnosis, with little appreciation given to the distinctions that characterize its multiple subtypes.9 More recently, idiopathic VT has been classified with respect to the ventricle of origin, the response to pharmacologic agents, evidence for catecholamine-dependence, and the morphologic features of the arrhythmia (QRS configuration and axis and whether repetitive, nonsustained, or sustained). A more mechanistically oriented classification also has been developed and is summarized in Table 1. As indicated in the table, the response of the arrhythmia to programmed stimulation and to adenosine, verapamil, and propranolol differentiates nearly all forms of idiopathic VT.
