Most mental disorders are treated in primary care, yet the classification that is best for psychiatrists does not answer the needs of clinicians in general health care, and as a result is very little used. A simplified primary care system for mental disorders was devised to accompany ICD-10, consisting of 24 conditions commonly occurring in these settings. While this has been a qualified success, the time has come to revise it. The considerations to be adopted in doing this in preparation for ICD-11 are described, which will provide broad correspondence with the main ICD, yet is likely to be used both in training and in countries with a wide range of medical services.
