La très grande majorité des intoxications par le monoxyde de carbone sont dues à l’inhalation du gaz issu d’une combustion. Nous rapportons un cas d’intoxication aiguë survenue sur le lieu de travail en l’absence de toute combustion. Le patient, technicien de maintenance, vérifiait des détecteurs de phosgène sur un site chimique industriel : il a présenté deux pertes de connaissance de durée brève. La mesure du monoxyde de carbone dans l’air expiré par l’infirmière de l’entreprise, grâce à un CO-testeur, a confirmé l’intoxication. Il a bénéficié d’une oxygénothérapie normobare prolongée à haut débit et d’une séance d’oxygénothérapie hyperbare. La synthèse du phosgène, gaz corrosif, est réalisée à partir du monoxyde de carbone, ce qui explique sa présence dans l’unité où intervenait le salarié.

Most carbon monoxide poisonings are due to the inhalation of the gas produced during combustion. We report one case of acute occupational poisoning without any combustion. The patient, a technician in the chemical industry, used to check phosgene detectors. He presented carbon monoxide poisoning with two short phases of loss of consciousness. The poisoning was confirmed in the infirmary at the work place using a CO-tester to measure carbon monoxide concentration in the expired breath. The treatment including 100% normobaric oxygen therapy for 12hours and one session of hyperbaric oxygen therapy at the hospital led to uneventful recovery. The synthesis of phosgene from carbon monoxide accounts for the presence of carbon monoxide in excess in the working unit.
