Encéphalopathie aiguë inhabituelle chez un jeune patient insuffisant chronique terminal - 29/06/13

La leucoencéphalopathie postérieure réversible (LEPR) représente un état neurotoxique rare qui peut être associé à différentes situations pathologiques. Son diagnostic repose sur un aspect radiologique caractéristique à l’imagerie par résonance magnétique encéphalique et l’évolution vers la résolution des symptômes en cas de prise en charge thérapeutique rapide et efficace. L’hypertension artérielle (HTA) et la prééclampsie en sont les principales étiologies. Les désordres métaboliques notamment l’insuffisance rénale peuvent également favoriser l’apparition de ce syndrome. Nous rapportons l’observation d’un jeune hémodialysé chronique âgé de 18 ans, admis en urgence pour un état de mal épileptique lié à une poussée hypertensive et apparu deux semaines après son transfert en dialyse péritonéale. L’imagerie par résonance magnétique (IRM) était en faveur d’une leucoencéphalopathie postérieure réversible compliquée d’hématomes intracérébraux. L’évolution était favorable avec la régression nette des lésions encéphaliques.

Posterior reversible encephalopathy represents a neurotoxic state which can be associated to different pathological situations. The key to diagnosis are characteristic features on magnetic resonance imaging and rapidly evolving neurological symptoms if recognized and treated rapidly. Arterial hypertension and preeclampsia are the main causes of posterior reversible encephalopathy. Metabolic disorders including renal failure can also be associated with this syndrome. We report the clinical observation of an 18-year-old young patient on chronic hemodialysis who presented a status epilepticus associated with hypertensive crisis, two weeks after his transfer to peritoneal dialysis. Magnetic resonance imaging showed a typical aspect of posterior reversible encephalopathy with intracerebral haemorrhage. The evolution was favorable with a clear improvement of encephalic lesions.
