Efficacité anti-amibienne in vitro de l’association riboflavine/UV-A - 19/03/15

Introduction |
Les kératites amibiennes constituent une pathologie rare, mais grave. Elles sont particulièrement difficiles à traiter lorsque le diagnostic est tardif, notamment en raison des limites des traitements actuels. Le but de notre étude est d’évaluer l’efficacité anti-amibienne de la riboflavine et des UV-A sur Acanthamoeba castellani.
Matériels et méthodes |
Nous avons testé l’effet de la chlorhexidine 0,02 % seule (C), de l’association riboflavine 1 % et UV-A (R+UV-A), et de la combinaison de ces deux procédures (C+R+UV-A) sur des cultures de formes végétatives et de kystes d’A. castellani. Nous avons procédé à un comptage parasitaire sous microscope optique de chaque zone traitée à 1, 4 et 8jours.
Résultats |
Il existe une diminution du nombre de kystes pour les trois traitements (C, R+UV-A, C+R+UV-A). Cette réduction est plus importante pour les géloses traitées par R+UV-A (p<0,01 à j8) et celles traitées par C+R+UV-A (p<0,001 à j8) en comparaison à celles exposées à la chlorhexidine seule (C). On ne note pas de diminution du nombre de trophozoïtes amibiens pour les trois traitements (C, R+UV-A, C+R+UV-A), mais un enkystement de ceux-ci.

Devant l’efficacité in vitro de l’application de riboflavine combinée aux UV-A contre les formes kystiques d’A. castellani et l’excellente tolérance in vivo de cette procédure, la prise en charge des kératites amibiennes pourrait être améliorée par cette nouvelle approche thérapeutique.

Introduction |
Acanthamoeba keratitis is a rare but serious disease and is particularly difficult to treat when the diagnosis is delayed, partly because of the limitations of current therapies. The purpose of our study is to evaluate the anti-amoebic effectiveness of riboflavin and UV-A on Acanthamoeba castellani.
Materials and methods |
We tested the effect of 0.02% chlorhexidine alone (C), the combination of riboflavin 1% and UV-A (UV-A+R), and the combination of the two treatments (R+C+UV-A) on cultures of vegetative and cystic forms of A. castellani. We conducted a parasite count under optical microscopy for each treated area at day 1, 4 and 8.
Results |
There was a decrease in the number of cysts for all three treatments (C, UV-A+R, R+C+UV-A). This reduction was greater for the plates treated with R+UV-A (P <0.01 at D8) and those treated with C+R+UV-A (P<0.001 at D8) compared to those exposed to chlorhexidine alone (C). There was no decrease in the number of amoebic trophozoites for the three treatments (C, UV-A+R, R+C+UV-A), but encystment was observed.
Discussion |
Given the in vitro efficacy of riboflavin combined with UV-A against cystic forms of A. castellani and excellent in vivo tolerance of the procedure, the treatment of acanthamoeba keratitis might be improved by this new therapeutic approach.
