Résumé |
Les spondylarthrites forment un groupe de rhumatismes voisins qui ont en commun de se manifester par une inflammation des synoviales et des enthèses. La synovite est peu spécifique et évoque d'assez près la synovite rhumatoïde. Leur particularité tient surtout à l'inflammation des enthèses, ou enthésite, dont le syndrome d'hyperostose–ostéite–périostite est la triade anatomoclinique. Le concept d'enthèse ne doit pas se limiter aux insertions osseuses des tendons, des ligaments ou des capsules articulaires, mais s'élargir à d'autres structures fibreuses comme les amphiarthroses (symphyses pubienne et manubriosternale, disque intervertébral) et les diarthro-amphiarthroses (région sacro-iliaque, articulations du thorax, extrémité des doigts et des orteils). Ces structures forment, avec les enthèses proprement dites, un vaste territoire qui est la cible privilégiée des spondylarthrites.

The term “spondyloarthritis” encompasses a variety of conditions that manifest as inflammation of the synovial membranes and entheses. The synovial inflammation lacks specificity and shares many features with rheumatoid synovitis. The hallmark of spondyloarthritis is inflammation of the entheses, which manifests as a combination of hyperostosis, osteitis, and periostitis. The entheses include not only the sites where tendons, ligaments, and joint capsules attach to bone, but also other fibrous structures such as the amphiarthroses (pubic symphysis, manubriosternal symphysis, and discovertebral junctions) and the diarthro-amphiarthroses (sacroiliac area, chest wall joints, and tips of the fingers and toes). This vast entheseal territory is the target selectively involved in patients with spondyloarthropathy.
