Astigmatisme interne et lésions oculaires associées - 30/03/12

L’astigmatisme est un défaut réfractif dont l’origine n’est pas toujours purement cornéenne, et correspond parfois à la résultante de contributions cornéenne, cristallinienne ou mixte. Le but de notre étude est de rapporter les lésions oculaires associées à un astigmatisme lenticulaire et leur évolution après traitement. Première observation : patient âgé de 25ans présente un astigmatisme extra cornéen unilatéral entrant dans le cadre du tilted disc syndrome associé à une myopie bilatérale. Notre patient a bénéficié d’un traitement par lasik. L’évolution postopératoire a été bonne avec un recul de cinq ans. Deuxième observation : patiente âgée de 35ans sans antécédents généraux présente un lenticône postérieur associé à un kératocône responsable d’un astigmatisme mixte important. Elle a bénéficié d’une chirurgie combinée : kératoplastie transfixiante avec extraction du cristallin et implantation d’un cristallin artificiel. L’évolution a été bonne avec bonne récupération visuelle. Le bilan d’un astigmatisme interne doit comprendre la réalisation systématique d’une réfraction subjective et objective sous cycloplégie et d’une topographie cornéenne. La constatation d’un astigmatisme interne est une indication à un bilan clinique et paraclinique détaillé. La recherche d’une étiologie est capitale pour dépister une pathologie oculaire ou générale associée et guider la stratégie thérapeutique. La connaissance de la puissance réfractive de la cornée et de l’astigmatisme cristallinien séparément serait importante pour la chirurgie réfractive et la chirurgie cristallinienne.

Astigmatism is a refractive defect whose origin is not always purely corneal, and is sometimes the result of corneal, crystalline lens or mixte contributions. The aim of our study is to report, through two cases, ocular lesions associated with a lenticular astigmatism and their evolution after treatment. In the first observation, it is a 25-year-old patient with a unilateral extra corneal astigmatism within the framework of the “tilted disc syndrome” associated with bilateral myopia. This patient has received treatment by Lasik. The postoperative course was good with a decline of 5ans. The second observation is that of a patient aged 35 years without having a general history with a posterior lenticonus associated with keratoconus responsible for a major mixed astigmatism. She received combined surgery: penetrating keratoplasty with lens extraction and implantation of an artificial lens. The evolution was good with good visual recovery. The balance of internal astigmatism must include the systematic achievement of a subjective and objective refraction under cycloplegia and corneal topography. The search for etiology is critical to screen for eye or general disease and guide the therapeutic strategy. Knowledge of the refractive power of the cornea and crystalline lens of astigmatism separately would be important for surgery refractive and crystalline lens surgery.
