État des lieux des services de gynécologie-obstétrique dans les centres hospitaliers publics français - 20/10/12

Les restructurations des services de chirurgie et de gynécologie-obstétrique qui s’opèrent actuellement soulèvent bon nombre d’interrogations. Nécessités incontournables pour les uns, fatalités des logiques économiques au détriment de la qualité et de l’accessibilité des soins pour d’autres, elles ont pour effet de clarifier bon nombre d’indicateurs socio-économiques et médicaux. Les plans de périnatalité pour le versant obstétrical et les seuils d’activité pour le versant chirurgical constituent des axes majeurs de ces restructurations. Une enquête sollicitant tous les centres hospitaliers français publics a permis d’apprécier l’état des lieux des services de gynécologie-obstétrique sous l’angle de ces récentes restructurations. La démographie médicale, la préservation et l’amélioration de la qualité et de la continuité des soins, l’efficience des plateaux techniques sont des critères discriminants des enjeux en présence. Ces restructurations ont un impact sur le vécu des médecins, qui apparaît globalement positif, ce qui est un bon présage pour parfaire cette démarche de remodelage. L’activité a été sauvegardée au prix d’une redistribution et d’un recentrage au sein des établissements. Il ne faut cependant pas minimiser l’impact social de ces changements, avec une potentielle dégradation des conditions de travail (reclassements professionnels internes, obligation de mobilité vers d’autres sites). Il apparaît ainsi que les profondes mutations dont font l’objet les établissements de taille « modeste » ne pourront bien s’opérer que si elles se font avec clarté (information) et s’intègrent véritablement dans leur projet médical.

Restructuring the surgery and gynecology-obstetrics departments taking place now raise many interrogations. It appears as a mandatory necessity to some people and as a tribute to financial strategies to others to the detriment of quality and accessibility of care. Its effect is to clarify a good amount of socioeconomical and medical indicators. The plans of perinatality for the obstetrical aspect and the thresholds of activity for the surgical aspect constitute the major lines of these restructurings. A survey soliciting all the French public hospitals was used to assess the state of obstetrics and gynecology departments in the light of these recent restructurings. Medical demography, preserving and improving the quality and continuity of care, efficiency of the technical supports are discriminating criteria of the involved challenges. Such restructurings have an impact on the doctor’s lives, which looks globally positive and a good omen to complete this remodeling process. The activity was safeguarded by a redistribution and a refocusing of institutions. One should not minimize the social impact of these changes, with a potential deterioration of working conditions (internal professional reclassifications, mobility obligation towards other sites). It thus appears that the deep changes which affect the small size institutions will be able to achieve well only if they are clearly done (information) and truly integrated in their medical project.
