Prise en charge des infections du post-partum - 05/12/12

L’endométrite du post-partum est fréquente et représente la cinquième cause de décès maternel. Cette pathologie est évitable et les traitements efficaces. Le streptocoque B est le principal pathogène incriminé. Le diagnostic clinique est facile et associe algies pelviennes, hyperthermie et des lochies fétides. En cas de non-réponse au traitement antibiotique ou s’il existe un doute sur une vacuité utérine, une échographie pourra être pratiquée. Un scanner ou une IRM seront réalisés devant une fièvre persistante malgré une antibiothérapie appropriée à la recherche d’un foyer profond nécessitant un drainage ou d’une thrombophlébite pelvienne. Celle-ci nécessite une héparinothérapie pendant la durée de l’antibiothérapie, voire par AVK pendant au moins trois mois en cas d’embolie pulmonaire ou d’extension à la veine cave. L’antibiothérapie de première intention associe la clindamycine à la gentamicine (une injection journalière) par voie intraveineuse. En cas de contre-indication ou d’allaitement maternel, d’autres molécules pourront être proposées. La durée idéale de l’antibiothérapie n’a pas pu être déterminée, le relais per os n’apportant pas de bénéfice. Concernant l’antibioprophylaxie (céphalosporines), celle-ci est indiquée en cas de césarienne au moment de l’incision cutanée alors qu’elle est discutée en cas d’accouchement assisté ou de déchirures périnéales des troisième et quatrième degrés.

Post-partum endometritis are frequent and account for the fifth cause of maternal death. This disease is preventable and effective treatments are available. Streptococcus agalactiae is the primary pathogen implicated. The clinical diagnosis is usually easy and involves pelvic pain, fever and abnormal lochia. Whenever antibiotic treatment provides no clinical improvement, or in case of doubt on the vacuity of the uterus, an ultrasound exam must be performed. Likewise, a CT scan or a MRI will be performed in case of persistent fever in search of a pelvic abscess requiring a drainage, or of a pelvic thrombophlebitis. Thromboplebitis requires heparin for the duration of antibiotic therapy, or oral anticoagulants for at least 3 months in case of pulmonary embolism or extension to the vena cava. The recommended antibiotic regimen combines clindamycin with gentamicin (once daily) intravenously. In case of contraindication or breastfeeding, other regimens may be prescribed. Adequate duration of treatment has not been evaluated and a switch to oral antibiotics after clinical improvement brings no benefit. Antibioprophylaxy (mostly cephalosporins) is recommended in cases of cesarean section (at skin incision), while it is debated in case of assisted delivery or of 3rd and 4th degree perineal tears.
