Évaluer les pratiques de prescription et d’administration des médicaments par sonde de nutrition entérale et améliorer les informations transmises au patient au retour à domicile.

Une évaluation des pratiques de prescriptions et d’administration chez les patients porteurs d’une sonde a été réalisée en 2013 en ORL, neurologie et neurochirurgie. À l’issue de l’enquête, une formation et des outils spécifiques ont été mis en place. Des entretiens pharmaceutiques de sortie ont été instaurés avec remise d’un plan de prise et d’un protocole d’administration. L’enquête a été réitérée en 2015 selon la même méthodologie, complétée d’une évaluation de la satisfaction des patients et des infirmiers libéraux.

Une réduction significative du taux de prescriptions inadaptées a été observée en 2015 : 11,5 % (25/218, 25 patients) vs 4,5 % (12/269, 27 patients) ; p<0,01. Il s’agissait principalement de formes à libération modifiée : 44,0 % en 2013 et 25,0 % en 2015 (p<10⁻⁵). Les formes buvables ont été d’avantage prescrites en 2015 : 25,7 % vs 16,5 % en 2013 (p<0,05). Le rinçage de la sonde avant administration des médicaments n’avait jamais été réalisé en 2013 alors qu’il était réalisé dans 75,0 % des cas en 2015 (p<0,05). Au total, 19 entretiens patients ont été réalisés, suscitant 35 interventions pharmaceutiques proposant une adaptation galénique. Tous les patients et les infirmiers libéraux ont jugé les documents pratiques et complets.

Cette démarche a induit une amélioration des pratiques. Les entretiens pharmaceutiques ont permis d’améliorer l’information fournie au patient et d’assurer un meilleur relais ville-hôpital.

Assessing both drug prescription and administration through enteral feeding tubes in order to optimize practices in hospital and at discharge.

The study aimed to evaluate prescriptions and administration through enteral feeding tubes in an ENT unit, neurological department and a neurosurgical intensive care unit. After a first study, nursing staff training was made and functional tools created. Pharmaceutical interviews were organized in ENT in patients with gastrostomy: drugs intake planning and administration procedure were given. This study was repeated in 2015 and patients and district nurses satisfaction was also evaluated.

Eleven percent of the prescribed medications were not suitable with enteral administration (modified-release tablets in 44.0% of cases). Prescription of unsuitable drugs was significantly reduced in 2015: 11.5% (n=218 lines of prescription, 25 patients) vs. 4.5% (n=269 lines of prescription, 27 patients); P<0.01. It concerned mostly modified-release drugs: 44.0% in 2013 and 25.0% in 2015 (P<10⁻⁵). Liquids formulations were more prescribed in 2015: 25.7% vs. 16.5% in 2013 (P<0.05). Feeding tube rinsing before drugs administration was never performed in 2013 and carried out by up to 75.0% of nurses in 2015. Nineteen pharmaceutical interviews were realized, leading to 35 pharmaceutical interventions to adjust galenics. All patients and district nurses found the documents informative and useful.

This survey was useful to improve nurses and physicians practices. Pharmaceutical interviews were useful to improve the information given to patients and the communication at discharge.
