Avec environ un tiers des nouvelles infections, les migrants représentent une population cible pour la prévention. En aval, une adaptation des pratiques des soignants est souhaitable pour améliorer leur prise en charge de façon à prendre en compte leurs spécificités (ce qui devrait être une règle générale) qui ne se limitent pas à des différences culturelles mais qui intègrent des déterminants socioéconomiques qu’il ne faut jamais perdre de vue. Cette adaptation des pratiques, au-delà d’une formation sur les droits des étrangers et sur des notions de base socioculturelles, doit passer par une plus grande disponibilité en temps et par une acceptation du « partage du pouvoir » en renforçant la relation médecin–malade, trop souvent exclusive, par d’autres intervenants prenant mieux en compte d’autres aspects de la vie des patients que la relation thérapeutique. Ainsi, la médiation apparaît être un outil très important, comme pour toute maladie chronique, dont on pourrait imaginer sans peine, sous des formes différentes, l’intérêt pour tous les patients. Le succès thérapeutique à long terme est probablement à ce prix, l’observance des patients migrants n’apparaissant pas différente de celle des autres patients.

Immigrants living in France account for one third of new cases of infection and are a target population for prevention. Care givers should adapt their management practice, taking into account this populationʼs specificities which are not restricted to cultural differences but include major socioeconomic factors. In addition to training on alien rights and basic sociocultural knowledge, care-givers (especially clinicians) must spend more time with the patient (especially at the beginning of the relationship) and accept sharing the “medical power” with other people with a better knowledge of other aspects of the patients’ life in addition to the medical one. As in other chronic diseases, mediation is one of the available tools with evident benefits for any patient. Assuming compliance is the same for migrants as for other patients, using this mediation will warrant therapeutic success.
