Adaptations ventilatoires du jeune asthmatique après un entraînement en créneaux : résultats préliminaires - 01/01/00

Riassunto

Objectifs. - Étudier les effets d'un programme d'entraînement en endurance chez de jeunes asthmatiques légers (une crise ou moins par semaine).

Méthodes. - Six jeunes (trois garçons et trois filles, 14 ± 2 ans, 49 ± 12 kg, 157 ± 6 cm) ont suivi ce programme, qui consistait en la répétition trois fois par semaine d'un exercice en créneaux de 45 minutes comportant la succession de neuf créneaux de cinq minutes. Chaque créneau est constitué par une base de 4 minutes et un pic d'une minute. Les intensités sont respectivement celle du seuil d'adaptation, puis de désadaptation ventilatoire, identifiés lors d'un test triangulaire préalable. L'intensité des bases et des pics était réajustée de 10 W chaque fois que la fréquence cardiaque mesurée aux 44e et 45e minutes avait baissé de huit à dix battements par minute par rapport à l'épreuve initiale. À l'issue d'un programme total de 20 séances, les effets de l'entraînement furent jugés à l'aide d'un second test triangulaire comparé au test initial.

Résultats. - La puissance maximale supportée (195 contre 152 W, p < 0,001) et la consommation maximale d'oxygène (51 contre 46,3 mL·min-1·kg-1, p < 0,05) sont significativement améliorées. Cet entraînement a également modifié l'adaptation ventilatoire des sujets : ceux-ci ont toléré une ventilation maximale supérieure, sans déclencher de crise d'asthme, tout en utilisant un plus grand pourcentage de leur ventilation maximale théorique (99 contre 86 %, p < 0,05). De même, les seuils d'adaptation ventilatoire (95 contre 64 W) et de désadaptation ventilatoire (152 contre 115 W) sont repoussés (p < 0,01).

Conclusion. - L'exercice en créneaux individualisé est bien supporté cliniquement. Le rapport bien ajusté de phases de travail intense (pics) et de phases de récupération active (bases) semble particulièrement efficace, en développant le potentiel aérobie et en augmentant la tolérance de ces jeunes asthmatique à l'hyperventilation d'exercice.

Abstract

Objective. - To study submaximal and maximal cardiorespiratory responses to exercise of mildly asthmatic teenagers (less than one asthma attack a week).

Methods. - Six subjects (14 ± 2 years, 49 ± 12 kg, 157 ± 6 cm) were assessed before and after 20 sessions (three days/week-1) of a 45-minute individualised interval training programme performed on a cycle ergometer. The training sessions consisted of a four-minute submaximal' base level at the workload corresponding to the anaerobic threshold, followed by a one-minute peak' level at the respiratory compensation point workload. The workloads were regularly increased throughout the study to reach the optimum metabolic stimulation.

Results. - The interval training adapted to each individual induced a marked increase of exercise tolerance (maximal workload: 195 versus 152 W, P < 0.001, and peak oxygen uptake. 51 versus 46.3 mL·min-1·kg-1, P < 0.05). Anaerobic threshold (95 versus 64 W) and respiratory compensation point (152 versus 115 W) are increased (P < 0.01). This training modified the subjects' maximal ventilatory adaptations. Subjects reached a higher minute ventilation and tidal volume, and used a higher percentage of their maximal voluntary ventilation (99 versus 86 %, P < 0.05) without developing an exercise-induced bronchoconstriction.

Conclusion. - The study demonstrates the efficiency and good tolerance of this individualised endurance training programme for asthmatic teenagers, without EIB. Such a programme may therefore provide additional benefits to the usual treatment of asthmatic dyspnea.
