A new in vivo confocal microscopy prognostic factor in Acanthamoeba keratitis - 24/02/14

Purpose |
To identify prognostic factors associated with poor outcomes in Acanthamoeba keratitis (AK).
Methods |
Patients with AK treated at the Beijing Tongren Hospital between January 2008 and January 2012 were included. All patients had corneal scrapings and/or cultures positive for Acanthamoeba and visible cysts on in vivo confocal microscopy (IVCM) examination. Therapeutic penetrating keratoplasty was performed in patients who experienced disease progression or lack of improvement on topical therapy. Patient demographics, clinical characteristics, previous treatment, and IVCM characteristics of the cysts were evaluated. Patients defined as poor outcomes were those requiring therapeutic penetrating keratoplasty. Logistic regression was used to estimate the odd-ratio identifying prognostic factors associated with a poor outcome.
Results |
Twenty-nine eyes of 29 patients were diagnosed as having AK over the study period. IVCM showed clusters and/or chains of Acanthamoeba cysts in 9 patients. Fifteen patients underwent therapeutic penetrating keratoplasty. A late-disease stage on presentation, a deep location of cysts, and clusters or chains of cysts observed with IVCM were significantly associated with a worse outcome. On multivariate analysis, only clusters or chains of cysts observed with IVCM were independently associated with a poor prognosis.
Conclusion |
The presence of clusters or chains of Acanthamoeba cysts could be a new IVCM criterion allowing the identification of AK patients requiring therapeutic penetrating keratoplasty.

Objectif |
Identifier les facteurs de mauvais pronostic associés aux kératites amibiennes.
Patients et méthodes |
Dans cette étude, les patients traités au Beijing Tongren Hospital pour une kératite amibienne entre janvier 2008 et janvier 2012 ont été inclus. Tous les patients avaient un examen direct après grattage de cornée et/ou des résultats de cultures positifs pour Acanthamoeba, et des kystes visibles en microscopie confocale in vivo (IVCM). Une kératoplastie transfixiante thérapeutique était réalisée chez les patients qui présentaient une évolution défavorable ou l’absence d’évolution sous traitement médical. Les données démographiques, les caractéristiques cliniques, les traitements antérieurs, et les caractéristiques des kystes amibiens observés en IVCM ont été analysés. La nécessité d’effectuer une kératoplastie transfixiante thérapeutique a été utilisée comme définition du mauvais pronostic. Une régression logistique a été réalisée afin d’identifier les facteurs de mauvais pronostic associés aux kératites amibiennes.
Résultats |
Vingt-neufs yeux de 29 patients atteints de kératite amibienne ont été analysés. L’examen en IVCM retrouvait des kystes amibiens groupés en amas ou en chaînettes chez 9 patients. Quinze patients ont nécessité une kératoplastie transfixiante thérapeutique. Lors de l’examen initial, un stade clinique avancé, une localisation profonde des kystes et la présence de kystes regroupés en amas ou en chaînettes en IVCM étaient associés à un pronostic défavorable. En analyse multivariée, seule l’observation en IVCM de kystes regroupés en amas ou en chaînettes était associée de manière indépendante à un plus mauvais pronostic.
Conclusion |
L’observation en IVCM de kystes amibiens regroupés en amas ou en chaînettes pourrait être un nouveau critère de mauvais pronostic des kératites amibiennes permettant d’identifier les patients nécessitant un traitement chirurgical.
