Primary hyperparathyroidism: comparison of MIBI-Tc99m parathyroid scintigraphy/SPECT-CT with histological results

Introduction |
L’hyperparathyroïdie primaire est une endocrinopathie fréquente dont la scintigraphie parathyroïdienne fait partie de son bilan préopératoire. Notre objectif était de confronter nos résultats de scintigraphie avec tomographie d’émission monophotonique couplée à un scanner (TEMP-TDM) aux données opératoires et notamment histologiques. .
Méthodes:Étude rétrospective portant sur 5ans, incluant 46 patients adultes opérés pour hyperparathyroïdie primaire et ayant une scintigraphie parathyroïdienne (double phase - double isotope et complétée par TEMP-TDM) positive et chez qui on dispose des comptes rendus opératoires et histologiques définitifs.
Résultats |
L’âge moyen était de 56,7ans avec un sexe ratio de 0,12. La parathormonémie (PTH) médiane était de 246 pg/mL avec une calcémie moyenne de 113,5mg/mL. Les sensibilités de l’échographie cervicale, de la scintigraphie planaire et de la TEMP-TDM étaient respectivement de 50 %, 89,12 % et 95,65 %. La TEMP-TDM avait une meilleure sensibilité chez les patients présentant un adénome infracentimétrique ou en position ectopique ou une thyroïde nodulaire. L’exploration chirurgicale était unilatérale dans 69,5 % des cas. La PTH peropératoire a baissé de plus de 50 % chez 95 % des patients. Quant à l’histologie, elle a conclu à un adénome parathyroïdien chez 45 patients. Le parenchyme parathyroïdien réséqué était normal chez un patient malgré l’aspect hypertrophique en peropératoire.
Conclusion |
La scintigraphie parathyroïdienne avec TEMP-TDM permet une localisation précise des glandes hyperfonctionnelles permettant ainsi d’adopter une chirurgie cervicale unilatérale ou mini-invasive et de diminuer le taux d’échec thérapeutique. Elle est également plus performante dans les glandes infracentimétriques et en cas de thyroïde nodulaire associée.

Background |
Primary Hyperparathyroidism is a common endocrinopathie whose parathyroid scintigraphy is part of its preoperative assessment. Our aim was to compare our results of scintigraphy with single-photon emission tomography coupled with a scanner (SPECT-CT) with the operative data and in particular histological data.
Methods |
Retrospective study including 46 patients who underwent surgery for hyperparathyroidism and had a positive parathyroid scintigraphy and in whom the definitive operative and histological reports are available.
Results |
The mean age was 56.7 years with a sex ratio of 0.12. The median parathyroid hormone (PTH) was 246 pg./ml with mean serum calcium of 113.5mg/ml. The sensitivities of cervical ultrasound, planar scintigraphy and SPECT-CT were respectively 50 %, 89.12 % and 95.65 %. A SPECT-CT had a better sensitivity in patients with a nodular thyroid or ectopic or infracentimetric adenoma. Surgical exploration was unilateral in 69.5 % of cases. Intraoperative PTH was reduced by more than 50 % in 95 % of patients. As for the histology, considered the gold standard diagnostic examination, it concluded that a parathyroid adenoma was found in 45 patients. The resected parathyroid parenchyma was normal in a single patient despite the hypertrophic appearance.
Conclusion |
The main contribution of SPECT-CT is the accurate localization of ectopic hyper functional glands, thus selecting patients for unilateral or minimally invasive cervical surgery and reducing the rate of therapeutic failure. It has also better results than planar scintigraphy in small parathyroid glands and in cases associated to nodular thyroids.
