Clinical reasoning has many different definitions subsuming many different processes and theories. Clinical reasoning can be generally conceived of as including both diagnostic reasoning and management reasoning. Safe, effective, efficient, and judicious diagnosis is the cornerstone of high-quality health care; yet, the diagnostic process is complex and fraught with opportunities for diagnostic error. In this article, we will discuss diagnostic reasoning’s theoretic underpinnings as well as factors leading to diagnostic error and strategies for improving diagnostic performance and enabling diagnostic excellence.
