La kératopathie neurotrophique (KNT) est une maladie secondaire à une diminution, voire à une abolition, de la sensibilité cornéenne par atteinte centrale ou périphérique de la branche ophtalmique du nerf trijumeau (V1). La défaillance des fonctions sensitives et trophiques se manifeste cliniquement par une hypo- ou anesthésie cornéenne, une raréfaction du clignement palpébral, une sécheresse oculaire et une atteinte épithéliale, qui peuvent conduire à des complications stromales ou à des retards de cicatrisation cornéenne. L'esthésiométrie cornéenne permet de quantifier l'hypoesthésie cornéenne et l'imagerie confocale confirme histologiquement l'atteinte nerveuse. Les étiologies sont nombreuses et concernent aussi bien les adultes que les enfants. La KNT peut aboutir à la perte anatomique et/ou fonctionnelle de l'œil atteint. Les objectifs de la prise en charge thérapeutique sont d'obtenir la cicatrisation des lésions cornéennes, de maintenir la qualité de vie et si possible de sauvegarder la fonction visuelle. Les traitements médicaux ou chirurgicaux mis en œuvre sont à adapter à la sévérité et au mode évolutif de la maladie. De nouvelles options thérapeutiques, comme la neurotisation cornéenne, visent à restaurer l'innervation sensitive de la cornée et permettent d'envisager depuis peu un possible traitement curatif de la KNT.

Neurotrophic keratopathy (NK) is a disease secondary to a decrease or abolition of corneal sensitivity secondary to central or peripheral damage to the ophthalmic branch of the trigeminal nerve (V1). Failure of sensory and trophic functions is clinically characterized by corneal hypo- or anesthesia, loss of eyelid blinking, dry eye, epithelial alterations leading to stromal complications and delayed corneal wound healing. Corneal esthesiometry allows corneal hypoesthesia to be quantified, and confocal imaging provides histological confirmation of nerve damage. The etiologies are numerous and concern adults as well as children. NK can lead to anatomical and/or functional loss of the affected eye. The goals of therapeutic management are to achieve healing of corneal lesions, maintain quality of life and, if possible, conserve visual function. The medical or surgical treatments implemented must be adapted to the severity and evolution of the disease. New therapeutic options, such as corneal neurotization, aim to restore sensory innervation of the cornea and have recently offered a potential cure for neurotrophic keratopathy (NK).
