Fibrinolyse intra-artérielle in situ chez une patiente monophtalme atteinte d’occlusion d’artère centrale de rétine - 23/06/09

Nous rapportons le cas d’une patiente monophtalme atteinte d’occlusion d’artère centrale de la rétine sur son œil fonctionnel. Il s’agit d’une patiente âgée de 72 ans ayant d’importants facteurs de risques cardio-vasculaires, forte myope bilatérale et amblyope de son œil droit. L’occlusion artérielle survenant sur l’oeil gauche, un traitement par fibrinolyse intra-artérielle in situ fut instauré par administration d’altéplase et d’héparine sodique dans l’artère ophtalmique gauche. Il permit à la patiente de retrouver son acuité visuelle initiale après un mois d’évolution, et ce, de façon stable après 9 mois de recul.

We report the case of a single-eyed patient with acute occlusion of the central retinal artery on her functional eye. The patient was a 72-year-old women with high cardiovascular risk, bilateral severe myopia, and amblyopia in her right eye. The occlusion affected the left eye so treatment consisting of local intra-arterial fibrinolysis was initiated by administration of alteplase and sodic heparin in the left ophthalmic artery. The result was successful restoration of initial visual acuity after 1 month, which was stable at the 9-month-follow-up.
