Sédation peropératoire à objectif de concentration avec le propofol : détermination des concentrations au site d'action et évaluation de l'index bispectral - 01/01/04

Résumé |
Objectifs. - Déterminer les concentrations au site d'action (Ce) de propofol pour assurer une sédation peropéatoire. Évaluer la fiabilité de la technique de sédation à objectif de concentration (SIVOC) ainsi que l'intérêt du BIS comme monitorage de la profondeur de sédation.
Type d'étude. - Étude clinique prospective ouverte.
Patients. - Patientes devant bénéficier de la pose de bandelettes sous-urétrales (TVT®) pour cure d'incontinence urinaire sous anesthésie locale.
Méthodes. - Après prémédication, la sédation peropératoire était assurée par le propofol administré avec le Diprifusor® à la concentration cible (Cc) initiale de 1 μg/ml. La Cc était ajustée toutes les cinq minutes par paliers de ± 0,2 μg/ml de façon à maintenir un niveau de sédation à 4 ou 3 sur l'échelle OAA/S (réponse à la stimulation verbale). La Ce de propofol et l'index bispectral (BIS) étaient enregistrés toutes les cinq minutes. Les relations entre BIS ou Ce et score OAA/S ont été analysées par régression linéaire et par le calcul de la probabilité de prédiction (PK).
Résultats. - Cinquante patientes âgées de 62 ± 12 ans ont été étudiées. Un score OAA/S à 4 ou 3 a été atteint chez toutes les patientes. La Ce et le BIS pour maintenir ce niveau de sédation étaient respectivement de 1,0 ± 0,2 μg/ml et 87 ± 7. Il existe une relation linéaire entre score OAA/S et BIS ou Ce mais avec une variabilité interindividuelle importante. La moyenne des PK obtenues chez chaque patiente est de 0,84 et 0,83 pour le BIS et Ce, respectivement.
Conclusion. - La SIVOC est une technique fiable pour assurer une sédation peropératoire. Elle permet une adaptation précise de la concentration de propofol en fonction de la réponse clinique du patient.

Objectives. - To determine the effect-site concentration (Ce) of propofol, required to achieving adequate sedation. To assess the efficacy and safety of a target-controlled infusion system during monitored anaesthesia care and to evaluate the ability of bispectral index (BIS) to predict sedation level.
Study design. - Prospective clinical study.
Patients. - Women scheduled for insertion of tension-free vaginal tape under local anaesthetic infiltration.
Methods. - After premedication with hydroxyzine, 1% propofol was infused using the Diprifusor® system at an initial target plasma concentration (Cc) of 1 μg/ml and then adjusted by steps of 0.2 μg/ml at 5 min intervals. The level of sedation was assessed using the observer's assessment of alertness/sedation (OAA/S) scale; the objective was to obtain an OAA/S level at 4 or 3 (response to verbal stimulation). Ce of propofol and BIS were noted every 5 min. Relation between Ce or BIS and OAA/S scale was analysed by linear regression and probability of prediction (PK).
Results. - Fifty patients aged 62 ± 12 years were studied. Sedation at level 4 or 3 was observed in all patients. Ce of propofol and BIS to maintain this OAA/S score were, respectively, 1.0 ± 0.2 μg/ml and 87 ± 7. There was a linear relation between OAA/S scale and BIS or Ce; however, individual values demonstrate wide variability. The average of PK values computed for each patient for the BIS and Ce was 0.84 and 0.83, respectively.
Conclusions. - Target-controlled infusion of propofol provides easy and safe management of intraoperative sedation, allowing a fast and precise adjustment of the propofol concentration to the clinical response of the patient.
