Techniques in ultrasonography have improved dramatically over the last several years. Ultrasonography provides patients with an excellent means of screening for anomalies, and the use of soft markers has individualized each patient's decision to pursue diagnostic testing. Consensus across disciplines, including radiology and obstetrics, is that a combined modality of maternal age, serum screening, and ultrasonography provides patients the best risk assessment. In the second trimester a normal ultrasonogram reduces the risk of trisomy 21 by 60% to 70% (likelihood ratio of 0.3–0.4) but does not completely negate it.
