Contexte La lutte contre la drogue et la prévention des dépendances constitue l’un des principaux objectifs de la circulaire DGS/DHOS n°2002-57 du 30 janvier 2002. Celle-ci autorise tout médecin exerçant en établissement de santé à initier un traitement par méthadone.

Objectifs Cette démarche a été mise en place afin de rendre plus accessible la méthadone aux toxicomanes. C’est dans ce cadre que la commission du médicament et des dispositifs médicaux stériles du centre hospitalier Philippe Pinel à Amiens (80) a mis en place des recommandations de prescription de l’initiation d’un traitement substitutif par méthadone.

Recommandations Elles précisent les conditions préalables à la prescription ainsi que le schéma d’utilisation à mettre en œuvre lors de la phase d’induction du traitement. Y figurent aussi les modalités de prescription, de surveillance du traitement et les précautions à observer. Enfin, dans le cadre d’une coopération ville-hôpital performante, les conditions de relais à mettre en œuvre pour la sortie du patient sont détaillées.

Conclusion Au-delà de simples protocoles de prescription, ces recommandations ont vocation à améliorer la prise en charge globale du patient toxicomane au sein d’un véritable réseau sanitaire.

Background The fight against drugs and the prevention of addiction is one of the principle aims of the information sheet DGS/DHOS n°2002-57 dated January 30, 2002. This text authorises any practitioner working in a health centre to initiate treatment with methadone.

Objectives This strategy was set-up in order to make methadone more accessible to drug addicts. It is within this context that the drug and sterile medical device commission of the Philippe Pinel hospital centre of Amiens (80) developed guidelines for the prescription and initiation of replacement therapy with methadone.

Guidelines They specify the conditions prior to the prescription as well as the regimen that must be applied during the initiation phase. The modalities of prescription, follow-up and precautions to be taken also figure in the document. Moreover, within the framework of an efficient town-hospital cooperation, the relay conditions to be applied on the release of the patient are detailed.

Conclusion Other than simple prescribing recommendations, these guidelines are aimed at improving the global management of drug addicts within the heart of an effective sanitary network.
