Les dégénérescences de l'iris et du corps ciliaire sont des affections rares de l'adulte. Les dégénérescences des tissus de l'iris sont dominées par le syndrome endothélial iridocornéen. L'étiologie reste débattue entre origine infectieuse ou inflammatoire. Il résulte de la prolifération et de la migration de cellules endothéliales cornéennes, ayant acquis des caractéristiques épithéliales (cellules endothéliales epithelium-like) en direction de la surface de l'iris. Elles envahissent l'angle iridocornéen. Le syndrome s'accompagne, à des degrés divers, d'œdème cornéen, de glaucome et de lésions rétractiles de l'iris, responsables d'atrophie et de la formation de trous. En fonction de la prédominance de l'atteinte des différentes structures, il est décrit trois entités différentes, l'atrophie progressive de l'iris, le syndrome de Chandler et le syndrome de Cogan-Reese. L'iridoschisis touche bilatéralement le stroma antérieur de l'iris qui se clive de l'épithélium pigmenté postérieur. Il se complique également d'hypertonie et d'altération de l'endothélium cornéen. Enfin, outre l'atrophie sénile de l'iris, de nombreuses atteintes oculaires (inflammation, infection, ischémie, traumatisme, myopie forte, glaucome aigu, iatrogènes, etc.) peuvent se compliquer de lésions dégénératives iriennes secondaires. Les dégénérescences du corps ciliaire sont dominées par les atteintes kystiques bénignes isolées ou de polykystose qui peut se compliquer de glaucome par son retentissement sur l'angle iridocornéen. Les dégénérescences malignes doivent également être éliminées. Dans ces pathologies, rétro-iriennes, la biomicroscopie ultrasonore a une place centrale pour le diagnostic, le suivi évolutif avant et après le traitement.

The degenerations of the iris and ciliary body are rare affections of the adult. The degenerations of the tissues of the iris are dominated by the endothelial iridocorneal syndrome. The etiology is still argued between an infectious or inflammatory origin. It results in the proliferation and migration of endothelial corneal cells having acquired the epithelial characteristics (endothelial epithelium-like cells) in the direction of the surface of the iris. They spread through the iridocorneal angle. The syndrome is accompanied, at various degrees, by corneal edema, glaucoma, retracted lesions of the iris, responsible for atrophy and the formation of holes. With respect to the predominance of the damage caused to the different structures, three different entities are described: progressive atrophy of the iris, the Chandler syndrome and the Cogan-Reese syndrome. The iridoschisis bilaterally affects the anterior stroma of the iris which is separated from the pigmented posterior epithelium. It is also complicated by hypertonicity and the alteration of the corneal endothelium. Finally, other than the senile atrophy of the iris, multiple ocular damages (inflammation, infection, ischemia, trauma, strong myopia, severe glaucoma, iatrogenic, etc.) can be complicated by secondary degenerative iridic injuries. The degenerations of the ciliary body are dominated by cystic benign isolated damages or by polycystic disease which can be aggravated with glaucoma by its impact on the iridocorneal angle. The malignant degenerations can be also be eliminated. In these retro-iridic pathologies, ultrasonic biomicroscopy has a central place in the diagnosis, the progressive follow-up before and after the treatment.
