Résumé

Les microangiopathies thrombotiques (MAT) sont dites secondaires quand elles s’associent à un contexte particulier responsable en général de leur survenue. Il s’agit principalement des MAT associées à une grossesse, un contexte d’allogreffe de cellules souches hématopoïétiques (CSH), un cancer, un médicament ou une infection par le VIH. Les MAT secondaires représentent un groupe hétérogène de pathologies dont la présentation clinique est très dépendante du contexte associé. Il est donc important de savoir reconnaître ces contextes puisqu’ils conditionnement la prise en charge et le pronostic de façon majeure. Les MAT secondaires représentent encore un défi en termes de prise en charge. Si des progrès importants réalisés dans la compréhension des MAT obstétricales ont permis d’en améliorer le traitement et le pronostic, d’autres formes comme les MAT en contexte d’allogreffe de CSH ou celles secondaires à une chimiothérapie restent de mauvais pronostic. L’étude des mécanismes physiopathologiques doit se poursuivre, en association également avec une approche plus empirique cherchant à évaluer l’intérêt de nouvelles thérapeutiques à niveau de preuve faible, qui peuvent parfois permettre a posteriori d’envisager les mécanismes physiopathologiques en jeu. Les résultats préliminaires encourageants observés avec l’éculizumab dans certaines de ces formes pourraient en représenter un exemple.

Abstract

Thrombotic microangiopathies (TMA) are termed secondary when associated to a specific context favouring their occurrence. They encompass mainly TMA associated with pregnancy, allogeneic hematopoietic stem cell transplantation, cancer, drugs, or HIV infection. Secondary TMA represent a heterogeneous group of diseases which clinical presentation largely depends on the associated context. It is therefore mandatory to recognize these conditions since they have a significant impact in TMA management and prognosis. A successful management still represents a challenge in secondary TMA. Significant progresses have been made in the understanding of pregnancy-associated TMA, allowing an improvement of prognosis; on the opposite, other forms of secondary TMA such as hematopoietic stem cell transplantation-associated TMA or TMA associated with chemotherapy remain of dismal prognosis. A better understanding of pathophysiology in these forms of TMA, in association with a more empirical approach through the use of new therapeutic agents that can also help in the understanding on new mechanisms a posteriori, should improve their prognosis. The preliminary encouraging results reported with complement blockers in this field could represent a convincing example.
