Le zona ophtalmique est la conséquence de la réactivation du virus varicelle-zona dans la partie ophtalmique du ganglion trigéminé. C'est une cause de morbidité importante chez les patients âgés, en raison des complications inflammatoires oculaires qui touchent près de la moitié des patients, et de neuropathies postzostériennes qui peuvent devenir handicapantes par leur intensité et leur chronicité. L'incidence du zona augmente fortement à partir de l'âge de 60 ans, mais il n'est pas rare chez les patients de 50 à 60 ans. Le risque total de développer un zona ophtalmique au cours d'une vie est de l'ordre de 1 à 4 %, mais il atteint près de 50 % chez les patients de plus de 85 ans n'ayant encore jamais été touchés par cette affection. L'arrivée des vaccins contre la varicelle et contre le zona devrait probablement permettre dans l'avenir de changer les données épidémiologiques. En attendant, la meilleure prise en charge continue de reposer sur la prescription précoce d'antiviraux et le traitement des douleurs pendant la phase aiguë, deux facteurs importants pour réduire le risque d'apparition de douleurs postzostériennes.

Zoster ophthalmicus is the consequence of the reactivation of the varicella-zoster virus in the ophtalmic region of the trigeminal ganglion. It is a cause of high morbidity in elderly patients because of ocular inflammatory complications that affect nearly half of the patients, and because of postzosterian neuropathies that can become a handicap by their intensity and chronicity. The incidence of shingles strongly increases as from the age of 60 years, but it is not uncommon in patients aged 50 to 60 years. The total risk of developing a zoster ophthalmicus in a lifetime is in the range of 1 to 4%, but it affects almost 50% of patients over 85 years old who have not been previously affected by this disease. The arrival of vaccines against varicella and zona is likely to change the epidemiological data in the future. Meanwhile, the best management continues to rely on early prescription of antivirals and treatment of pain during the acute phase, two important factors to reduce the risk of the postzosterian pain.
