La tomographie en cohérence optique (OCT) a été la révolution de l'imagerie oculaire des dernières années. Initialement développé pour mesurer l'épaisseur maculaire dans le suivi des œdèmes maculaires diabétiques, cet appareil s'est révélé être l'outil indispensable pour une analyse quasi histologique de l'œil. En effet, en utilisant un faisceau lumineux balayant la rétine (ou autres structures), l'OCT permet d'établir une image selon différents degrés de réflectivités correspondant aux différentes couches cellulaires avec une résolution actuellement autour de 5 à 10 μm. De plus, cette technique possède quasiment tous les avantages : non invasif, rapide, fiable, reproductible et délégable. Ce qui explique son formidable développement et sa généralisation à une activité ophtalmologique spécialisée mais également générale. La France est probablement le pays au monde le plus équipé en OCT, ce qui fait des ophtalmologistes français des utilisateurs chevronnés. Pour en retirer le maximum, il convient de bien connaître les principes techniques et les différentes évolutions ainsi que les points-clés pour la réalisation et l'interprétation de l'examen. Optical coherence tomography (OCT) has been the eye-imaging revolution of recent years. Initially developed to measure macular thickness in the monitoring of diabetic macular edema, this device has proved to be the indispensable tool for an almost histological analysis of the eye. Indeed, by using a light beam scanning the retina (or other structures), the OCT allows to establish an image according to different degrees of reflectivities corresponding to the different cell layers with a resolution currently around 5 to 10 μm. In addition, this technique has almost all the advantages: non-invasive, fast, reliable, reproducible and delegable. This explains its tremendous development and its generalization to a specialized but also general ophthalmological activity. France is probably the world's most equipped country in OCT, which makes French ophthalmologists experienced users. To get the most out of it, you need to know the technical principles and the different developments as well as the key points for carrying out and interpreting the examination. This article is the update of “Technique and interpretation of optical coherence tomography”.
