Introduction

The goal of this study was to measure by spectral domain optical coherence tomography (SD-OCT) with EDI the choroidal thickness in healthy subjects and to compare these parameters with those of patients with retinitis pigmentosa (RP).

Methods

Data were obtained from 60 healthy patients without history or family history of retinal or choroidal disease or glaucoma. A case-control study was also conducted on 40 eyes of 20 patients with RP and 40 eyes of 20 healthy refraction- and age-matched controls, selected from among the 60 healthy patients. OCT was used with the EDI protocol. The primary outcome measure was choroidal thickness.

Results

Among healthy patients, the overall choroidal thickness was 287.7μm. Mean choroidal thickness was lower on the nasal side (236.6μm at 2000μm from the fovea) compared with the temporal side (262.3μm at 2000μm, P=0.002). It also varied according to age, being highest among 20–29-year-old patients and decreasing thereafter with increasing age. Choroidal thickness was significantly higher in healthy patients than in RP patients, regardless of the location (P<0.001).

Conclusion

This observational study confirms that choroidal thickness varies with age and location. It decreases in subjects with RP and is related to worsening of retinal damage, independently of age-related thinning. Further studies are needed to understand whether choroidal vascular alteration is a cause or a consequence of the degenerative pathology.

Introduction

L’objet de ce travail est de mesurer par tomographie en cohérence optique en spectral domain (SD-OCT) avec EDI l’épaisseur choroïdienne dans un groupe de sujets sains et de comparer ces paramètres avec ceux de patients atteints de rétinite pigmentaire (RP).

Méthodes

Les données ont été obtenues à partir de 60 sujets sains sans antécédent de glaucome ou de pathologie rétinienne ou choroïdienne. Une étude cas-témoin a également été conduite sur 40 yeux de 20 patients avec RP et 40 yeux de 20 des 60 sujets sains appariés en âge et réfraction. Le critère de jugement principal était l’épaisseur choroïdienne.

Résultats

Parmi les sujets sains, l’épaisseur choroïdienne centrale était de 287,7μm. L’épaisseur moyenne était plus faible du côté nasal (236,6μm à 2000μm de la fovéa) que du côté temporal (262,3μm à 2000μm, p=0,002). Elle diminuait avec l’âge, étant plus élevée chez les sujets de 20–29 ans que chez les sujets plus âgés. L’épaisseur choroïdienne était significativement plus importante chez les sujets sains que chez les patients atteints de RP quelle que soit la localisation (p<0,001).

Conclusion

Cette étude observationnelle confirme que l’épaisseur choroïdienne varie en fonction de l’âge et de la localisation. Elle diminue chez les sujets atteints de RP, et est liée à l’aggravation de l’atteinte rétinienne, indépendamment de l’amincissement lié au vieillissement. D’autres études sont nécessaires afin de comprendre si la modification de la vascularisation choroïdienne est une cause ou une conséquence de la pathologie dégénérative.
