Summary

Purpose

To evaluate baseline predictive markers of early and late anatomical response to anti-vascular endothelial growth factor (anti-VEGF) treatment in patients with neovascular age-related macular degeneration (nAMD).

Methods

The records of the nAMD patients who underwent intravitreal ranibizumab or aflibercept treatment, received the 3 monthly loading doses, and completed a follow-up period of 12 months were included retrospectively. The anatomical treatment response at month 3 (early) and between month 3 and 12 (late) was classified as good, intermediate or poor. Baseline demographic, fluorescein angiography, and optical coherence tomography findings were compared among the three groups.

Results

One hundred and ten eyes (74.3%) showed good, 18 (12.2%) showed intermediate and 20 (13.5%) showed poor anatomical response at month 3, and 114 eyes (77.0%) showed good, 27 (18.2%) showed intermediate and 7 (4.7%) showed poor anatomical response between month 3 and month 12. Of the evaluated parameters, drug type (better in aflibercept), showed a statistically significant difference in regards to anatomical outcomes at both the early and late periods (P=0.02 and P=0.03). The greatest linear dimension of choroidal neovascularization (CNV) and presence of peaked pigment epithelial detachment (PED) were important factors for early anatomical anti-VEGF treatment response.

Conclusion

Larger CNV and the presence of a peaked PED appeared to be associated with a good early response, and the drug type seemed to be associated with both early and late poor anatomical response of anti-VEGF treatment in nAMD patients. Aflibercept appears to be more effective than ranibizumab in regards to the percentage of patients with better anatomical response in both the early and late treatment periods.

Objectif

Évaluer les marqueurs prédictifs initiaux de la réponse anatomique initiale et tardive au traitement antifacteur de croissance endothélial vasculaire (anti-VEGF) chez les patients atteints de dégénérescence maculaire liée à l’âge néovasculaire (DMLA).

Méthodes

Les patients atteints de DMLA qui ont subi un traitement intravitréen de ranibizumab ou d’aflibercept et qui ont reçu les 3 doses mensuelles de charge, et complété une période de suivi de 12 mois, ont été inclus rétrospectivement. Les réponses anatomiques au traitement des patients au 3ème mois (la réponse initiale) et entre le 3e et le 12e mois (la réponse tardive) ont été classées comme bonne, intermédiaire ou mauvaise. Les données démographiques initiales, l’angiographie à la fluorescéine et la tomographie par cohérence optique ont été comparées entre les trois groupes.

Résultats

Cent dix yeux (74,3 %) ont montré une bonne réponse, 18 (12,2 %) yeux une réponse intermédiaire et 20 (13,5 %) yeux une mauvaise réponse anatomique initialement au 3e mois. 114 yeux (77,0 %) ont montré une bonne réponse, 27 (18,2 %) yeux une réponse intermédiaire et 7 yeux une mauvaise réponse anatomique entre les 3e et 12e mois. Parmi les paramètres évalués, le type de médicament (mieux dans l’aflibercept) a montré une signification statistique en ce qui concerne la réponse initiale et tardive (P=0,02 et P=0,03). La plus grande dimension linéaire de la néovascularisation choroïdienne (NVC) et la présence d’un décollement épithélial pigmentaire (DEP) avec une zone de pointe étaient les facteurs importants pour la réponse anatomique initiale au traitement d’anti-VEGF.

Conclusion

La plus grande NVC et la présence d’une zone de pointe du DEP étaient associées avec une bonne réponse initiale et le type de médicament était associé avec une réponse anatomique pauvre à la fois au début et à la fin du traitement d’anti-VEGF chez les patients atteints de DMLA. L’aflibercept semble être plus efficace que le ranibizumab en ce qui concerne le pourcentage de patients ayant une meilleure réponse anatomique à la fois au début at à la fin du traitement.
