Summary

Purpose

To investigate the position of the mental foramen based on its relation with the roots of the mandibular teeth and quantify the prevalence of anterior loop and satellite foramina in the mandible in cone beam computed tomography (CBCT) scans.

Methods

The sample consisted of 400 CBCT scans of males (n=190) and females (n=210) from Moscow, Russia. Using the system proposed by Tebo and Telford (1950), the position of the mental foramen was classified into: I) mesial to the mandibular first premolar; II) at the apex of the mandibular first premolar; III) between the roots of the mandibular first and second premolars; IV) at the apex of the mandibular second premolar; V) between the roots of the mandibular second premolar and first molar; and VI) at the apex of the roots of the mandibular first molar. Additionally, the images were assessed to investigate the prevalence of the anterior loop of the mandibular canal and the presence of satellite foramina.

Results

Mental foramen position class III was the most prevalent (61%) followed by class IV (27%), II (8%), I, V and VI (together <4%). Statistically significant differences were not detected between males and females (P<0.05). In Russian males and females, the anterior loop of the mandibular canal was found in 15.78% and 3%, respectively; while satellite foramina were found in 31.58% and 19.62%, respectively.

Conclusion

The spatial position of the mental foramen and the morphological alterations of the mandibular canal and adjacent foramina must be known prior to surgical interventions in the mandible.

Résumé

L’objectif était d'analyser par cone beam CT (CBCT) la position du foramen mentonnier par rapport aux racines des dents mandibulaires et quantifier la prévalence boucle antérieure et de la présence de foramina satellites.

Matériel et méthodes

L’échantillon était composé de 400 CBCT de patients masculins (n=190) et féminins (n=210). Se basant sur la méthode de Tebo et Telford (1950), la position du foramen mentonnier a été classée dans les groupes suivants : I : mésial par rapport à la première prémolaire mandibulaire ; II : à l’apex de la première prémolaire mandibulaire ; III : entre les racines de la première et de la deuxième prémolaire ; IV : à l’apex de la deuxième prémolaire mandibulaire ; V : entre les racines de la deuxième prémolaire mandibulaire et de la première molaire ; et VI : à l’apex des racines de la première molaire. De surcroît, les images ont permis d’analyser l’existence de la boucle antérieure du canal mandibulaire et la présence de foramina satellites.

Résultats

La classe III s’avère prédominante (61 %), suivie des classes IV (27 %), II (8 %), enfin des classes I, V et VI (qui, ensembles, ne représentent que moins de 4 %). Aucune différence significative n’a été détectée entre les sujets masculins et féminins (p<0,05). Dans les deux genres (masculin, féminin), la boucle antérieure du canal mandibulaire a été observée respectivement dans 15,78 % et 3 % des cas, tandis que les orifices satellites ont été visualisés respectivement dans 31,58 % et 19,62 % des cas.

Conclusion

La position du foramen mentonnier et les diverses variantes anatomiques représentent des données importantes dont il faut tenir compte avent toute intervention à visée thérapeutique.
