Le malate de sunitinib inhibe plusieurs récepteurs à activité tyrosine kinase impliqués dans la croissance tumorale, l'angiogenèse pathologique et la progression métastatique du cancer. Les indications reconnues du sunitinib sont le cancer rénal métastatique ou non resécable chirurgicalement et les tumeurs stromales gastro-intestinales après échec de l'imatinib. Des études prospectives et rétrospectives ont montré une association entre l'utilisation du sunitinib et l'apparition d'une hypothyroïdie touchant plus de 50 % des malades traités dans certaines séries. Plus intrigante a été la disparition de l'image thyroïdienne à l'échographie dans deux cas. Le mécanisme de cette atteinte thyroïdienne est encore inconnu. Il pourrait s'agir d'une thyroïdite destructrice, mais aucune preuve d'auto-immunité favorisant cet événement n'a été apportée. D'autres explications possibles seraient l'action anti-angiogénique du sunitinib ou son activité antithyroperoxydase. Parce que les dysthyroïdies sont accessibles au traitement, il faut les dépister par un bilan tous les trois mois afin d'améliorer la qualité de vie de ces patients. Le sunitinib étant destiné à traiter des maladies mettant en jeu le pronostic vital, l'apparition d'une hypothyroïdie ne doit pas faire interrompre le traitement. La « destruction thyroïdienne échographique » pourrait faire suspecter d'une action antitumorale thyroïdienne du sunitinib.

Sunitinib inhibits numerous tyrosine kinase receptors involved in tumor growth, angiogenesis, and metastatic invasion. It is indicated in case of metastatic renal carcinomas and gastrointestinal stromal tumors (GIST) resistant to imatinib. Prospective and retrospective studies have shown association between use of sunitinib and hypothyroidism affecting more than 50% of patients in some series. More amazing, was the non-visualisation of thyroid tissue evaluated with thyroid ultrasonography in two cases. Mechanisms of this side effect are not elucidated. Some studies have suggested destructive thyroiditis but no evidence of autoimmunity has been demonstrated. Anti angiogenic effect could be another hypothesis. Recently antithyroperoxidase activity of sunitinib has been demonstrated. Because hypothyroidism is easily accessible to treatment, screening of thyroid abnormalities is mandatory every three months to improve quality of life of these patients. This unique thyroid side effect of sunitinib with the non-visualisation of thyroid suggests a possible and promising antitumor activity in thyroid cancer.
