Résumé

La nanomédecine a connu un développement important depuis les années 2000 et ce n’est que très récemment que deux nanoparticules métalliques ont fait leur apparition dans des essais cliniques chez l’homme. Le mécanisme d’action de ces agents radiosensibilisants repose sur la présence d’atomes avec un numéro atomique (Z) élevé permettant un dépôt de dose plus important dans la tumeur lors de l’irradiation. La première nanoparticule qui a été utilisée chez l’homme est NBTXR3, composée d’hafnium (Z=79), en injection intratumorale dans le traitement des sarcomes. Une autre nanoparticule à base de gadolinium (Z=64), l’AGuIX, a été utilisée en injection intraveineuse dans le traitement des métastases cérébrales. Les premiers résultats cliniques sont prometteurs en termes de faisabilité, de tolérance et d’efficacité en témoigne le nombre importants d’essais cliniques en cours. Les enjeux actuels pour le développement des nanoparticules résident dans : leur capacité de ciblage des cellules cancéreuses, leur biodistribution dans l’organisme, leur toxicité éventuelle et leur production industrielle. Dans les années à venir, les schémas d’administration et les meilleures combinaisons possibles avec la radiothérapie devront être définies en lien avec la recherche fondamentale.

Abstract

Nanomedicine has undergone significant development since the 2000s and it is only very recently that two metallic nanoparticles have emerged in clinical trials. The mechanism of these radiosensitizing agents is based on the presence of atoms with a high atomic number (Z) allowing a higher dose deposition into the tumor during irradiation. The first nanoparticle used in humans is NBTXR3, composed of hafnium (Z=79), with intratumor injection for the treatment of sarcoma. Another gadolinium-based nanoparticle (Z=64), AGuIX, has been used for intravenous injection in the treatment of brain metastases. The preliminary results are promising in terms of feasibility, safety and efficacy, as evidenced by the significant number of ongoing clinical trials. The upcoming challenges for the development of nanoparticles will be the targeting of cancer cells, their biodistribution into the body, their eventual toxicity and their industrial production. In the coming years, modalities of administration and optimal combinations with radiotherapy should be defined in connection with fundamental research.
