Les alopécies dites biphasiques sont mal connues et très peu d’articles y font référence. Une alopécie est dite biphasique lorsqu’au décours de son évolution les follicules pileux sont détruits alors qu’elle est initialement considérée comme non cicatricielle. Les alopécies que l’on peut considérer comme potentiellement biphasiques sont les alopécies d’origine traumatique par traction ou pression, la pelade et l’alopécie androgénétique. Le rapprochement entre clinicien et anatomopathologiste est dans ce domaine particulièrement utile. L’examen anatomopathologique, réalisé sur des coupes horizontales, permet d’apporter les arguments nécessaires au diagnostic, de donner des éléments pronostiques, d’orienter la thérapeutique. Les raisons de cette évolution biphasique sont discutées, la chronicité d’un processus inflammatoire étant la cause prépondérante. Pour certains auteurs, toute alopécie évoluant sur le long terme deviendrait cicatricielle.

So-called “biphasic alopecia” is a poorly known entity referred to in very few articles. Alopecia is termed “biphasic” where hair follicles are destroyed despite the process initially involving no scarring. Forms of alopecia that may be considered potentially biphasic are those of traumatic origin caused by traction or pressure, alopecia areata, and androgenetic alopecia. Close cooperation between the clinician and the histopathologist is particularly valuable in this area. The histopathological examination, carried out on horizontal sections, can provide the elements necessary to enable diagnosis, support a prognosis, and guide therapy. Herein the reasons for this biphasic course are discussed, with chronic inflammatory processes being the main cause. For some authors, all long-term forms of alopecia eventually become cicatricial.
