Le parcours de soins du patient insuffisant cardiaque est complexe et nécessite une coordination pluridisciplinaire. Un projet pilote a été initié avec l’objectif d’évaluer l’intérêt de la conciliation médicamenteuse dans le parcours du sujet âgé insuffisant cardiaque au sein de deux services de soins. Au total, 85 patients ont bénéficié d’une conciliation à l’entrée et 41 à la sortie. Pour plus d’un sur deux, une divergence non-intentionnelle (DNI) était retrouvée lors de la prescription d’entrée. À la sortie, dix DNI ont été interceptées et corrigées. Le document de conciliation de sortie était systématiquement envoyé aux professionnels de ville. La collaboration médecin-pharmacien et pharmacien hospitalier-pharmacien d’officine a permis le partage d’informations et donc la sécurisation de la prise en charge médicamenteuse aux points de transition du parcours de soins du patient.

The care pathway for patients with heart failure is complex and requires multidisciplinary coordination. A pilot project was launched to evaluate the benefits of medication reconciliation in the care of elderly heart failure patients in two care units. A total of 85 patients were conciliated on admission and 41 on discharge. For more than one in two, an unintentional discrepancy (DNI) was found at the time of admission. At discharge, ten DNIs were intercepted and corrected. The discharge conciliation document was systematically sent to city professionals. Collaboration between doctor-pharmacist and hospital pharmacist-pharmacist enabled information to be shared, thereby ensuring the safety of drug management at transition points in the patient’s care pathway.
