La prothèse de hanche de resurfaçage (RH) conjugue un double questionnement, à savoir bien entendu la confirmation de l’intérêt porté à ce concept prothétique de « préservation du capital osseux », mais également le bien-fondé de la méthodologie pour la mise en place et la gestion d’une collecte exhaustive de données cliniques, sous la forme d’un véritable « registre national observationnel du resurfaçage de hanche »…

Il faut souligner dans le début des années 2000, les fortes incertitudes formulées quant à l’avenir de ce resurfaçage de hanche. Dans ce contexte, le CNP-COT s’était prononcé en faveur du maintien de cette option prothétique, sous couvert de la mise en route d’un registre observationnel exhaustif en 2015 et de la création d’un Centre Formateur agréé pour la France. Ce registre français du resurfaçage de hanche est donc une étude observationnelle, non comparative, prospective, multicentrique, à 1 bras, menée en ouvert avec inclusion exhaustive chez des patients traités consécutivement par arthroplastie totale de la hanche avec une prothèse de resurfaçage.

L’exhaustivité des patients inclus dans le registre de resurfaçage est vérifiée par comparaison, sur la période d’inclusion considérée, d’une part, (1) entre le détail des facturations pour les différents implants RH livrés aux différents centres poseurs et, d’autre part, (2) pour chacun des chirurgiens participant à l’étude, à leurs inclusions dans le registre RRH au sein de la base de données en ligne OrthoWave™.

Les objectifs fixés par les autorités de tutelle (HAS, CNP-COT) et la Société française de la hanche et du genou (SFHG) ont été validés, avec un taux d’exhaustivité de 93,47 % lors de la constitution du dossier HAS, et un taux de survie au-delà de 98 % pour 3592 prothèses à 8 ans de recul. À noter par ailleurs un taux de complications très faible (<1 %) avec notamment absence de luxations ou de phénomènes allergiques aux ions métalliques.

Le fait marquant de cette étude souligne l’intérêt de cette prise en charge d’un registre observationnel exhaustif spécifique au sein de la Société française de chirurgie hanche et genou, ce qui a permis de pérenniser la possibilité d’implantation des resurfaçages de hanche en France.

The resurfacing hip prosthesis (RH) combines a double questioning, namely of course the confirmation of the interest in this prosthetic concept of “preservation of bone capital”, but also the merits of the methodology for the implementation place and management of an exhaustive collection of clinical data, in the form of a true “national observational register of hip resurfacing”…

It should be noted in the early 2000s that there was considerable uncertainty about the future of hip resurfacing. In this context, the CNP-COT was in favor of maintaining this prosthetic option, under cover of the launch of an exhaustive observational register in 2015 and the creation of an approved Training Center for France. This French hip-resurfacing registry is therefore an observational, non-comparative, prospective, multicenter, 1-arm study, conducted open-label with exhaustive inclusion in patients treated consecutively by total hip arthroplasty with a resurfacing prosthesis.

The completeness of the patients included in the resurfacing register is verified by comparison, over the inclusion period considered, on the one hand (1) between the details of invoicing for the different RH implants delivered to the different fitting centers and on the other part (2) for each of the surgeons participating in the study, to their inclusion in the RRH register within the OrthoWave™ online database.

The objectives set by the Supervisory Authorities (HAS, CNP-COT) and the French Hip and Knee Society (SFHG) were validated, with a completeness rate of 93.47% when the HAS file was created, and a survival rate of over 98% for 3592 prostheses after 8 years. Note also a very low complication rate (<1%) with notably absence of dislocations or allergic phenomena to metal ions.

The highlight of this study underlines the interest of this management of a specific exhaustive observational register within the French Society of Hip and Knee Surgery, which made it possible to perpetuate the possibility of implanting hip resurfacing in France.
