Résumé

Contexte

L’influence du tabagisme sur le risque d’infection articulaire périprothétique (IAP) reste incertaine. L’objectif était d’explorer l’impact du tabagisme sur l’IPP après une arthroplastie totale primaire du genou (ATG) et de la hanche (ATH).

Hypothèse

Les patients fumeurs devraient avoir un risque accru d’IPJ par rapport aux patients non-fumeurs.

Patients et méthodes

Une étude de cohorte observationnelle prospective basée sur un registre a été réalisée. Un total de 4591 patients ayant subi une ATG primaire (3076 patients) ou une ATH (1515) ont été inclus. Selon le statut tabagique au moment de l’arthroplastie, les patients ont été classés comme non-fumeurs (3031 patients), ex-fumeurs (688) et fumeurs (872). L’analyse multivariée a pris en compte le statut tabagique, l’âge, le sexe, le niveau d’éducation, l’indice de masse corporelle, la classe de l’American Society of Anesthesiologists, le diagnostic (arthrose, rhumatisme), le diabète, la broncho-pneumopathie chronique obstructive, la transfusion sanguine périopératoire, le site de l’arthroplastie (genou, hanche), la durée de l’opération et la durée du séjour.

Résultats

Il y a eu des IPJ après 59 (1,9 %) ATG et 27 (1,8 %) ATH (p=0,840). Il y a eu des IPJ chez 47 (1,6 %) non-fumeurs, 12 (1,7 %) ex-fumeurs et 17 (1,9 %) fumeurs (p=0,413). Des complications de la plaie (retard de cicatrisation et infection superficielle de la plaie) ont été observées chez 34 (0,7 %) non-fumeurs, 9 (1,3 %) ex-fumeurs et 17 (1,9 %) fumeurs (p=0,045). Dans l’analyse multivariée, seul le sexe féminin était un facteur prédictif significatif d’IPJ (OR 1,3, IC 95 % : 1,1–2,4 [p=0,039]). Plus précisément, les catégories des ex-fumeurs (OR 0,8, IC 95 % : 0,2–1,7 [p=0,241]) et des fumeurs (OR 1,1, IC 95 % : 0,6–1,5 [p=0,052]) n’étaient pas des facteurs prédictifs significatifs. La survie à 4ans après arthroplastie avec une IPJ comme critère de jugement était de 99,1 % (IC 95 % : 99,0–99,7) pour les non-fumeurs, 99,0 % (IC 95 % : 98,8–99,2) pour les ex-fumeurs et 98,7 % (IC 95 % : 98,2–99,0) pour les fumeurs, sans différence significative entre les groupes de tabagisme (p=0,318).

Discussion

Le tabagisme n’a pas été identifié comme un facteur prédictif significatif de PJI après une ATG ou une ATH primaire.

Summary

Background

The influence of smoking on the risk of periprosthetic joint infection (PJI) remains unclear. The objective was to explore the impact of smoking on PJI after primary total knee (TKA) and hip (THA) arthroplasty.

Hypothesis

Current smoking patients should have an increased risk of PIJ compared with nonsmoking patients.

Patients and methods

A prospective registry-based observational cohort study was performed. A total of 4591 patients who underwent primary TKA (3076 patients) or THA (1515) were included. According to the smoking status at the time of arthroplasty, patients were classified as nonsmokers (3031 patients), ex-smokers (688), and smokers (872). Multivariate analysis included smoking status, age, gender, education level, body mass index, American Society of Anesthesiologists class, diagnosis (osteoarthritis, rheumatism), diabetes, chronic obstructive pulmonary disease, perioperative blood transfusion, site of arthroplasty (knee, hip), length of operation, and length of stay.

Results

There were PJI after 59 (1.9%) TKA and 27 (1.8%) THA (P=0.840). There were PJI in 47 (1.6%) nonsmokers, 12 (1.7%) ex-smokers, and 17 (1.9%) smokers (P=0.413). There were wound complications (delayed wound healing and superficial wound infection) in 34 (0.7%) nonsmokers, 9 (1.3%) in ex-smokers, and 17 (1.9%) in smokers (P=0.045). In multivariate analysis, only the female gender was a significant predictor of PJI (OR 1.3, 95% CI: 1.1–2.4 [P=0.039]). Specifically, the categories of ex-smokers (OR 0.8, 95% CI: 0.2–1.7 [P=0.241]) and smokers (OR 1.1, 95% CI: 0.6–1.5 [P=0.052]) were not significant predictors. The 4-year arthroplasty survival with PJI as the endpoint was 99.1% (95% CI: 99.0–99.7) for nonsmokers, 99.0% (95% CI: 98.8–99.2) for ex-smokers, and 98.7% (95% CI: 98.2–99.0) for smokers was not significantly different between smoking status groups (P=0.318).

Discussion

Smoking was not identified as a significant predictor for PJI following primary TKA or THA.
