Résumé

Objectif

Évaluer le niveau de preuve des recommandations d’antibiothérapie pour les soins primaires.

Méthode

Après recueil des guides de pratique clinique (GPC) listés sur le site Antibioclic® ( antibioclic.com/ ) pour chaque recommandation portant sur la prescription ou non d’un antibiotique, une évaluation a été réalisée sur la mention ou non du GRADE de la Haute autorité de santé (HAS) : A, B, C, ou avis d’expert (AE). Les sources citées par les GPC ont été étudiées. Les recommandations et études devaient répondre à la question « faut-il ou non prescrire une antibiothérapie dans cette situation ? ». Les réponses possibles étaient « oui », « non », ou « oui conditionnel », c’est-à-dire, prescription différée. Le critère de jugement principal était le pourcentage de recommandations pour lesquelles des études étaient citées, comparativement au GRADE indiqué.

Résultats

Au total, 152 recommandations issues de 49 GPC sur la prescription d’antibiotiques ont été analysées. Au total, 71,7 % ne mentionnaient pas de niveau de preuve, 3,3 % étaient classées de GRADE A et 9,2 % de GRADE B. En examinant les études justifiant les recommandations, des preuves de haute qualité ont été identifiées pour 7,9 % des recommandations, tandis que 80,9 % d’entre elles n’avaient pas d’études référencées.

Conclusion

La plupart des recommandations françaises sur la prescription d’antibiotiques pour les soins primaires ne fournissent pas le niveau de preuve les justifiant.

Summary

Objective

To assess the level of evidence supporting antibiotic therapy recommendations in primary care.

Method

After collecting the clinical practice guidelines (CPGs) listed on the Antibioclic® website ( antibioclic.com/ ), each recommendation regarding whether or not to prescribe antibiotics was evaluated based on whether it included a GRADE classification by the French National Authority for Health (HAS): A, B, C, or Expert Opinion (EO). The sources cited by the CPGs were reviewed. The recommendations and studies had to answer the question: “Should antibiotic therapy be prescribed in this situation?” The possible answers were “yes,” “no,” or “conditional yes,” meaning delayed prescription. The primary outcome was the percentage of recommendations for which supporting studies were cited, in comparison to the stated GRADE.

Results

A total of 152 recommendations from 49 CPGs on antibiotic prescription were analyzed. In all, 71.7% did not mention a level of evidence, 3.3% were classified as GRADE A, and 9.2% as GRADE B. Upon reviewing the studies used to justify the recommendations, high-quality evidence was identified for only 7.9% of the recommendations, while 80.9% had no referenced studies.

Conclusion

Most French recommendations regarding antibiotic prescription in primary care do not provide the level of evidence supporting them.
