Résumé

La pneumopathie interstitielle diffuse (PID) est une atteinte fréquente et potentiellement létale de la sclérodermie systémique, touchant 40 à 50 % des patients. Le dépistage systématique par scanner thoracique haute résolution et épreuves fonctionnelles respiratoires (EFR) avec DLCO est recommandé dès le diagnostic. Une pneumopathie interstitielle diffuse (PID) peut être présente malgré des EFR normaux. Le pronostic dépend de l’étendue initiale de la PID, définie par l’imagerie et la capacité vitale forcée (CVF). Les recommandations actuelles (PNDS, ACR/CHEST, EULAR) préconisent une prise en charge personnalisée basée sur le risque évolutif. Le mycophénolate mofétil (MMF) est considéré comme traitement de première ligne. Le rituximab (RTX), évalué dans plusieurs essais, montre un effet favorable sur la stabilisation ou l’amélioration de la CVF. Le tocilizumab a également démontré un ralentissement du déclin de la CVF dans les sous-groupes avec PID dans deux essais randomisés, mais sans effet sur le critère principal basé sur le score cutané de Rodnan. Le nintédanib a prouvé son efficacité dans l’essai SENSCIS, réduisant significativement le déclin annuel de la CVF, y compris chez les patients recevant un MMF. En cas de progression malgré un traitement initial bien conduit, une autogreffe de cellules souches hématopoïétique ou une combinaison MMF + RTX peut être discutée. La transplantation pulmonaire peut être envisagée dans les formes réfractaires. Une approche intégrative associant oxygénothérapie, réhabilitation et prophylaxie infectieuse est essentielle.

Abstract

Interstitial lung disease (ILD) is a frequent and potentially life-threatening complication of systemic sclerosis, affecting 40 to 50% of patients. Systematic screening with high-resolution computed tomography and pulmonary function tests (PFTs), including DLCO, is recommended at diagnosis. ILD may be present even in patients with normal PFTs. Prognosis depends on the initial extent of ILD, assessed by imaging and forced vital capacity (FVC). Current guidelines (PNDS, ACR/CHEST, EULAR) recommend a personalized management approach based on progression risk. Mycophenolate mofetil (MMF) is considered the first-line treatment. Rituximab (RTX), evaluated in several trials, has shown a favorable effect on FVC stabilization or improvement. Tocilizumab has also demonstrated a slowing of FVC decline in ILD subgroups in two randomized trials, although the primary endpoint based on the modified Rodnan skin score was negative. Nintedanib demonstrated efficacy in the SENSCIS trial by significantly reducing the annual rate of FVC decline, including in patients receiving MMF. In cases of disease progression despite appropriate initial treatment, hematopoietic stem cell transplantation or a combination of MMF and RTX may be considered. Lung transplantation can be an option for refractory forms. A comprehensive approach including oxygen therapy, pulmonary rehabilitation, and infection prophylaxis is essential.
