Summary

Introduction

Arteriovenous malformations are high-flow vascular malformations linked to the presence of abnormal connections between feeding arteries and draining veins. They are frequently located in the head and neck, with major functional and morphological repercussions.

Materials and methods

Retrospective and descriptive study of patients operated on in our department for craniofacial arteriovenous malformations.

Results

Six patients: five male and one female, with lesions located in various areas: scalp, eyelid, infratemporal fossa and cheekbone. Treatment was standardised with preoperative embolisation and surgical excision of the arteriovenous malformation, except for two patients: one admitted as an emergency case for a ruptured AVM and the other for a superficial frontal AVM with surgically controllable feeding vessels, who were treated exclusively by surgery. The outcomes were straightforward with no signs of complications, and the loss of tissue was reconstructed using various repair techniques.

Discussion

AVMs are congenital high-flow malformations, but may become apparent later in life and sometimes after trauma. They have a highly polymorphic presentation: red or purplish skin and subcutaneous swelling, sometimes keratotic, warm, pulsatile with a murmur and sometimes a “thrill”; the course is unpredictable, sometimes stabilising and sometimes worsening, which can quickly become life-threatening for the patient. The diagnosis is radiological and clinical, and treatment is often based on a combination of embolisation and surgery.

Conclusion

The ultimate complication of AVMs is heart failure, which is a sign of slow progression. We recommend early treatment to prevent complications from occurring.

Résumé

Introduction

Les malformations artérioveineuses sont des malformations vasculaires de haut débit liées à la présence des communications anormales entre des ou d’une artère nourricière et des ou d’une veine de drainage, la localisation au niveau tête et cou est fréquente avec un retentissement fonctionnel et morphologique majeur.

Matériel et méthode

Étude rétrospective et descriptive portant sur les patients opérés dans notre service pour une malformation artérioveineuse crânio-faciale.

Résultats

Six patients : 5 hommes et une femme, différentes localisations sont constatées : cuir chevelu, paupière, fosse infra-temporale et jugal, la prise en charge était standardisée avec une embolisation préopératoire et une chirurgie d’exérèse carcinologique, sauf pour deux patients : un admis dans le cadre d’urgence pour rupture d’une MAV et pour l’autre pour MAV frontale superficielle avec des vaisseaux nourriciers contrôlables chirurgicalement qui ont été pris en charge exclusivement par chirurgie, les suites étaient simples, la reconstruction des pertes de substances était assuré par différentes techniques de réparation.

Discussion

Les MAV sont des malformations haut débit, congénitales, mais peuvent se révéler tardivement au cours de la vie et parfois en post-traumatique. Elles ont une présentation très polymorphe : tuméfaction cutanée et sous-cutanée rouge ou violacée, parfois kératosique, chaude, pulsatile avec un souffle et parfois un « thrill », l’évolution est imprévisible parfois stabilisation et parfois une aggravation qui peut rapidement engager le pronostic vital du patient. Le diagnostic est radio-clinique et le traitement repose souvent sur l’association de l’embolisation et de la chirurgie.

Conclusion

La complication ultime des MAV est l’insuffisance cardiaque signe d’une évolution lente, nous recommandons la prise en charge à un stade précoce pour éviter la survenue des complications.
