Abstract

Psychiatric nosology serves both clinical and research needs by providing a common language and informing healthcare planning. However, a movement toward transdiagnostic approaches is growing for assessments and care management of neurodevelopment disorders. During childhood and adolescence, symptoms appear as developmental delays. They often overlap across disorders, making the diagnosis difficult. To account for the multidimensional symptomatology, we propose a visualization of neurodevelopmental disorders phenomenology that would make it easier to analyze an individual as a network of diagnostic criteria whose interactions can compensate for each other and determine a pathological, at-risk or healthy impact. Such spatial organization would allow for the diagnostic criteria to be ordered over time, prioritized, and shared more effectively beyond the categorial diagnosis. Symptoms for neurodevelopmental disorders were grouped according to a hierarchy of phenomenological and symptomatological terms found in the main reference manuals and in the didactic consensus reported in the latest international reviews. They fall into four major domains: cognition, emotion, behavior and motricity while the operational diagnostic criteria were organized into a multiscalar hierarchy with a notion of order following a circular tree structure: the ‘sunburst’. A graphic, visual, dynamic and hierarchical organization was proposed to handle all the phenomenology terms used in children, adolescents and young adults (aged 5–25) more easily and to convey them to the parties involved. Far from being exhaustive, this visual representation aims to facilitate the use of psychiatric phenomenology while maintaining a critical, dynamic and evolving approach to scientific knowledge and clinical practice.

Résumé

La nosologie psychiatrique répond aux besoins cliniques et de recherche en fournissant un langage commun et en éclairant la planification des soins. Aujourd’hui, émergent de plus en plus des approches trans-diagnostiques d’évaluation et de prise en charge des troubles du neurodéveloppement. Les symptômes et dimensions cliniques de ces troubles se manifestent par des retards de développement dès l’enfance, et beaucoup se chevauchent, rendant le diagnostic et le suivi clinique souvent difficile. Au-delà du diagnostic catégoriel, considérer la multi-dimensionnalité de la symptomatologie, permettrait d’assurer une prise en charge plus individualisée. Afin d’appréhender ces dimensions dans la pratique clinique, nous proposons une visualisation de la sémiologie des troubles du neurodéveloppement. Cette visualisation faciliterait l’analyse d’un individu comme un réseau de critères diagnostiques dont les interactions peuvent se compenser et déterminer un impact pathologique, à risque ou sain. Une telle organisation spatiale permettrait également de mieux ordonner les critères diagnostiques dans le temps, de les hiérarchiser et de les partager plus efficacement. Les symptômes des troubles neurodéveloppementaux ont été regroupés selon une hiérarchie de termes sémiologiques et symptomatologiques issus des principaux ouvrages de référence et consensus didactique rapporté dans les dernières revues internationales. Les symptômes se répartissent en quatre grands domaines: cognition, émotion, comportement et motricité. Les critères diagnostiques sont organisés selon une hiérarchie multiscalaire, avec une structure arborescente circulaire: le «  sunburst  ». Une organisation graphique, visuelle, dynamique et hiérarchique a été proposée afin de faciliter la compréhension et la transmission des termes sémiologiques utilisés chez l’enfant, l’adolescent et le jeune adulte (5–25 ans). Loin d’être exhaustive, cette représentation visuelle vise à simplifier l’utilisation de la sémiologie psychiatrique de l’enfant et l’adolescent tout en adoptant une approche critique, dynamique et évolutive des connaissances scientifiques et de la pratique clinique.
