Abstract

Electroconvulsive therapy (ECT) is an effective intervention for treatment-resistant schizophrenia, but failure to induce adequate seizures remains a clinical challenge, especially in patients with high seizure thresholds. We report the case of a 55-year-old woman with chronic treatment-resistant schizophrenia and a left temporal meningioma, who initially responded to bifrontal ECT but later developed resistance to induced seizures. A switch to right unilateral (RUL) stimulation successfully re-induced therapeutic seizures, with sustained clinical improvement. To understand this outcome, we conducted electric field modeling using patient-specific MRI data and finite-element simulations with SimNIBS. The analysis compared bifrontal and RUL montages, with and without modeling of the meningioma. Results showed no significant impact of the meningioma on current distribution. However, the RUL montage consistently generated higher mean electric fields in cortical regions compared to the bifrontal configuration. These findings offer a mechanistic explanation for the clinical efficacy of the RUL approach in this case and highlight the potential of electric field modeling to optimize stimulation parameters, especially in anatomically complex cases. This case supports the utility of switching electrode montages as a simple, safe, and effective strategy when seizure thresholds rise, and it suggests a promising role for computational modeling in guiding personalized ECT planning. Further research is needed to assess the broader applicability of this approach and to explore its cognitive and functional implications.

Résumé

L’électro-convulsivothérapie (ECT) est une thérapie efficace pour traiter la schizophrénie résistante au traitement. L’incapacité d’induire des crises d’épilepsie adéquates est un défi clinique, particulièrement chez les patients présentant un seuil élevé à l’induction de ces crises. Nous présentons le cas d’une patiente de 55 ans souffrant de schizophrénie réfractaire associée à un méningiome temporal gauche. Initialement répondeuse à l’ECT avec un montage bi-frontal, elle a ensuite développé une résistance à l’induction des crises. Un changement du montage bi-frontal à unilatéral droit a permis de réinduire des crises adéquates, avec une amélioration concomitante de l’état clinique. Une analyse du champ électrique basée sur l’IRM de la patiente, utilisant SimNIBS, a été conduite comprendre les causes de ce changement. Cette analyse a comparé les deux montages employés, avec et sans prise en compte du méningiome. Cette analyse n’a pas montré d’influence du méningiome sur la distribution du courant. En revanche, cette analyse a montré que les densités de courant étaient, en moyenne plus hautes dans les régions corticales pour le montage unilatéral droit que pour le montage bi-frontal. Ces découvertes offrent une possible explication mécanistique de l’efficacité du montage unilatéral dans ce cas. Elles démontrent également le potentiel de ce type d’analyse pour l’optimisation des paramètres de stimulation, particulièrement dans des cas neuro-anatomiques complexes. Le cas clinique présenté illustre également l’utilité clinique d’un changement de montage, la présentant comme une stratégie simple, sans danger et efficace lorsque les seuils de provocation des crises épileptiques augmentent. Ce travail montre également le rôle prometteur de la modélisation numérique dans la planification et le guidage personnalisés de l’électro-convulsivothérapie. D’autres travaux sont nécessaires pour évaluer l’applicabilité de cette approche ainsi que ses implications cognitives et fonctionnelles.
