Résumé

Dans de nombreux pays des zones tropicales et subtropicales, le scorpionisme est un véritable problème de Santé publique. Le nombre d'espèces dangereuses est limité, mais des études récentes montrent que ce nombre est sous-estimé. Les venins de scorpions sont avant tout neurotoxiques et cardiotoxiques. Après une piqûre, la douleur est immédiate, violente et durable. Les autres symptômes de l'envenimation apparaissent après un intervalle de deux heures environ, la période critique se situant de la troisième à la quinzième heure. Passé vingt-quatre heures, le pronostic vital n'est habituellement plus en jeu. La guérison, rapide, est obtenue sans séquelles dans la très grande majorité des cas, sauf rarissimes exceptions.

Dans un petit nombre de cas, le risque vital est en jeu, principalement chez les sujets jeunes, enfants et adolescents, et d'autant plus que le sujet piqué est plus jeune. Le traitement spécifique (sérothérapie) est à mettre en œuvre aussi rapidement que possible. Le traitement symptomatique vise à calmer la douleur dans les cas bénins, à maintenir les grandes fonctions vitales dans les cas graves. La piqûre par un scorpion dangereux d'une taille supérieure à 5 cm, chez un sujet jeune, à la saison chaude, doit être considérée comme une urgence et conduire à une hospitalisation en soins intensifs. La diminution des fabrications de sérums anti-venimeux est un problème préoccupant.

Abstract

In many tropical and subtropical countries, scorpionism is a real Public health problem. A few species are considered as dangerous for humans, but some recent studies have shown that the risk is underestimated. The scorpion venoms are neurotoxic and cardiotoxic.

The sting is immediately very painful. The other signs of envenomings appear after a delay of about two hours, the critical period being comprised between the third and the fifteenth hour. After one day, it is generally admitted that the vital prognosis is no more engaged. Then, the recovery is rapid without any sequelæ, except very rare observations.

In a very few number of cases, there is a vital risk for children and adolescents, the risk being inversely proportional to the age. The specific treatment (serotherapy) must be carried out as rapidly as possible. The specific treatment aim at soothing the pain, at maintaining the vital functions in severe cases. A sting of a child by a dangerous adult scorpion (length more 5 cm) in the hot season has to be considered as a therapeutic emergency. The reduction of the antivenomous sera production is a preoccuping problem.
