Le groupe de réflexion éthique d’un centre régional de lutte contre le cancer présente son activité à partir d’une analyse a posteriori d’un cas clinique. L’observation concerne un patient avec une dyspnée réfractaire survenant en phase terminale. L’équipe médico-soignante relate un climat de tension et d’incompréhension avec les proches. Les différents aspects d’une situation complexe gérée dans l’urgence sont analysés à partir de points de vue médicaux, soignants, juridiques, sociologiques et psychologiques.

The activity of a working group on ethical questions in a regional center for the prevention of cancer is presented via a post hoc analysis of a medical case. The case observed is that of a patient with terminal cancer in a palliative care unit suffering from refractory dyspnea. The medical team relates the tension and communication difficulties with the family which mark this complex emergency situation. The various factors which had to be taken into account are analyzed from the standpoints of the doctors and the nurses involved and from a legal, sociological and psychological point of view.
