Épaule aiguë hyperalgique atypique : intérêt de l’échographie - 23/08/10

Résumé |
Nous rapportons un cas d’épaule aiguë hyperalgique dont la présentation clinique était particulière et pour laquelle l’échographie a permis d’en préciser le mécanisme et d’apporter une solution thérapeutique. La patiente était non seulement incapable de mobiliser son épaule mais elle ne pouvait non plus effectuer des mouvements de flexion et d’extension de l’avant-bras. L’échographie a montré que la calcification provoquait un œdème tendineux et subluxait le tendon du chef long du biceps hors de sa gouttière. L’infiltration locale de corticoïde sous contrôle échographique a permis en sept jours une diminution de l’œdème du subscapulaire et un retour du tendon du chef long du biceps dans sa gouttière.

A patient presented with an atypical pattern of acute severe shoulder pain. Sonography elucidated the mechanism of the pain and allowed effective treatment. The patient was unable not only to move her shoulder but also to flex and to extend her elbow. Sonography showed a calcific deposit in the subscapularis tendon with local edema displacing the long head of the biceps tendon out of the bicipital groove. Local injection of a glucocorticoid under ultrasonographic control was followed within seven days by subsidence of the subscapularis tendon edema and by a return of the long head of the biceps tendon to its normal position in the bicipital groove.
