Aneurysmal artery within a gastric ulcer - 17/08/11
| Commentary In 1829, Cruveilhier described the appearance of a blood vessel protruding from the floor of a gastric ulcer, but it was Swain and colleagues in 1986 (Gastroenterology 1986;90:595-608) who emphasized its prognostic significance. He pointed out that the structure protruding from the ulcer either may be a visible vessel or a clot plugging the vessel and described aneurysmal dilatation in 14 of 27 cases. The presence of the aneurysm explains why direct endoscopic therapy may precipitate arterial bleeding and supports the principle of treating the area surrounding the vessel without necessarily disturbing the aneurysm.
