| Commentary This case violates the clinical adage that when you hear hoof beats, you should bet on horses. Of course, gallstones can erode into the duodenum and the stomach, and present with massive upper-GI bleeding, but these are rare occurrences. One could question whether pneumobilia could have been observed earlier and, therefore, the diagnosis made sooner, but retrospectroscopy is always 20/20.
