Double balloon discovers double capsules - 23/08/11

| Commentary Double-balloon endoscopy, double capsule…I would have expected this patient to have a duplication. But diverticulosis of the small bowel is not a bad second choice, in the sense of an alternate route (Lat: deverticulum, a bypass). In fact, the small bowel is the second place that diverticula occur in large number, the first, of course, being the colon. Jejunoileal diverticulosis is seen in 0.5% to 2.3% of small bowel series, and usually they are asymptomatic. In this patient, they also were asymptomatic, except they provided a nidus within which the capsules could lodge. This, in fact, is one of the ways these diverticula cause problems, eg, when vegetable matter lodges there and forms an enterolith, subsequently, to cause bleeding, perforation, or obstruction. Fortunately, the capsules were retrieved before and without adverse incident. My guess is that only 1 bill was submitted for services rendered, despite the likelihood that the endoscopist had twice as much fun getting the 2 capsules out as he would have had with just 1.
