Current understanding of ejaculation is limited. In 1786 Hunter expressed curiosity about the process of ejaculation and its associated disorders in his description of the normal ejaculate, observing that “the semen that constitutes the first part of the ejaculate is evidently different from the last.”25 Recent advances have broadened knowledge of this physiologic process that is critically important for reproductive fitness. This review describes the normal process of ejaculation, its disorders, and their associated treatments.
