Introduction |
Les infections de prothèse aortique sont une complication rare de la chirurgie vasculaire. Nous en rapportons un cas, associé à une fistule duodénoparaprothétique, au cours duquel est survenue une bactériémie à Actinomyces odontolyticus.
Exégèse |
Il s'agit d'un homme de 73 ans, porteur d'une prothèse aortobifémorale et d'une parodontopathie, hospitalisé pour fièvre inexpliquée. Les hémocultures sont revenues positives à Escherichia coli et A. odontolyticus. L'imagerie était en faveur d'une infection de la prothèse. Le diagnostic a été confirmé au cours d'une laparotomie exploratrice qui a mis en évidence une infection polymicrobienne et une fistule duodénoparaprothétique. Une revue de la littérature des infections de prothèse aortique est effectuée. Le rôle d'A. odontolyticus dans cette infection est discuté.
Conclusion |
Les infections de prothèse aortique dues à une fistule prothéto-intestinale sont habituellement polymicrobiennes et représentent une complication tardive de la chirurgie aortique. Les différentes techniques d'imagerie sont essentielles au diagnostic des infections de prothèse aortique. La participation d'A. odontolyticus à l'infection de prothèse dans cette observation est possible.

Prosthetic vascular graft infection is a rare complication of vascular surgery. We report a case with graft enteric fistula and Actinomyces odontolyticus bacteremia.
Exegesis |
A 73 year-old man with a prosthetic aortic graft and who had a parodontal disease, has been hospitalised for fever of unknown origin. Blood cultures grew with Escherichia coli and Actinomyces odontolyticus. The imaging studies indicated graft infection. Laparotomy has confirmed the diagnosis and highlighted a polymicrobial infection and a paraprosthetic duodenal fistula. A review of the literature's data concerning prosthetic vascular graft infections is made. The role of Actinomyces odontolyticus in that case is discussed.
Conclusion |
Prosthetic aortic graft infection due to graft enteric fistula is usually a polymicrobial infection and is a late complication of aortic surgery. Imaging is essential for the diagnosis of prosthetic aortic graft infection. It is possible that Actinomyces odontolyticus has contributed to prosthesis infection in this case.
