Résumé

Objectif

Nous rapportons deux cas de décollement choroïdien massif apparus sous collyre bimatoprost.

Observations

Deux patients âgés respectivement de 95 (cas 1) et 74ans (cas 2) nous ont été adressés pour décollement choroïdien massif. Le cas 1 était monophtalme et l’œil atteint du cas 2 avait été opéré de trabéculectomie deux ans auparavant. Les deux yeux atteints étaient pseudophakes et traités depuis six mois par l’association fixe de bimatoprost et timolol pour un glaucome chronique à angle ouvert (cas 1) et pour un glaucome sur pseudoexfoliation capsulaire (cas 2). Dans les deux cas, les symptômes avaient débuté immédiatement après l’introduction du collyre et s’étaient progressivement aggravés depuis lors. Dans les deux yeux affectés, on notait une hyperhémie conjonctivale modérée, l’examen du segment antérieur et la pression intraoculaire étaient normaux, l’examen du fond d’œil révélait un décollement choroïdien massif. Ce diagnostic a été confirmé par échographie en mode B dans les deux cas. Nous avons suspecté un effet secondaire du bitamoprost et le collyre a été interrompu conduisant à la résorption complète du décollement choroïdien dans les trois semaines.

Conclusion

Le décollement choroïdien est un effet secondaire rare qui n’est pas spécifique du bimatoprost mais qui a également été rapporté avec les autres molécules de la famille des analogues des prostaglandines. Sa cause exacte reste incertaine mais implique probablement à la fois l’action du collyre sur la voie uvéosclérale et son effet pro-inflammatoire. L’arrêt de l’analogue des prostaglandines conduit à la résolution complète et définitive du décollement choroïdien dans les trois semaines.

Summary

Purpose

To report two cases of choroidal detachment induced by topical bimatoprost.

Observations

Two males, aged 95 (case 1) and 74 years (case 2), were referred to our department for massive choroidal detachment. Case 1 was a one-eyed patient, and the affected eye of case 2 had undergone trabeculectomy 2 years previously. Both affected eyes were pseudophakic and treated for 6 months with topical bimatoprost/timolol fixed combination for open angle glaucoma (case 1) and pseudoexfoliation glaucoma (case 2). In both cases, symptoms appeared immediately after starting the drops and progressively worsened thereafter. In both affected eyes, moderate conjunctival hyperemia was noted, anterior segment examination and intraocular pressure were normal, and fundus examination revealed a massive choroidal detachment. This diagnosis was confirmed by B-scan ultrasonography. An adverse effect of bimatoprost was suspected, and the drug was discontinued, leading to the complete resolution of the choroidal detachment within 3 weeks in both eyes.

Conclusion

Choroidal detachment is a rare adverse effect which is not specific to bimatoprost but which has been also reported with the other prostaglandin analog drops. Its exact cause remains unclear, but is probably linked to the combination of enhanced uveoscleral outflow and pro-inflammatory properties. In all cases, the discontinuation of the prostaglandin analog led to the complete resolution of the choroidal detachment within 3 weeks.
