Résumé

Le dermatofibrosarcome est une tumeur mésenchymateuse de malignité intermédiaire, récidivante, rare et atteignant surtout les adultes jeunes. De façon rare mais non exceptionnelle, il peut survenir chez l’enfant. Peu de séries rapportent sa prise en charge chez l’enfant. Le traitement curatif est chirurgical via une exérèse large latéralement et en profondeur. Nous avons étudié 15 cas de dermatofibrosarcomes chez des enfants âgés de sept à 18ans entre 1995 et 2008. Il s’agit d’une étude rétrospective qui a pour objectif de préciser la prise en charge du dermatofibrosarcome chez l’enfant. L’âge moyen des patients au moment du diagnostic est de 13ans. Dans tous les cas, les patients n’ont bénéficié d’aucun traitement chirurgical préalable. Le diagnostic a été confirmé histologiquement sur biopsie et marquage en immunohistochimie. Dans 60 % des cas, la lésion, particulièrement polymorphe, siégeait essentiellement sur le tronc. Dans trois cas, un traumatisme précédant l’apparition de la lésion était rapporté par le patient. Le traitement chirurgical a consisté en une exérèse avec une marge latérale de 3cm et emportant la première barrière anatomique saine en profondeur. La reconstruction s’est faite selon la localisation et l’importance de la perte de substance induite latéralement, en profondeur et selon sa topographie. Le délai de suivi moyen est de 7,8ans. Aucune récidive n’est à signaler à trois ans au moins. Les complications secondaires à la chirurgie ont été deux désunions ayant nécessité une greffe de peau mince de couverture et une alopécie cicatricielle qui a fait l’objet d’un protocole d’expansion secondaire. Aucun des patients ne présente de séquelle fonctionnelle du traitement chirurgical. La tumeur, plus rare chez l’enfant, a les mêmes caractéristiques cliniques et évolutives que chez l’adulte. Un diagnostic précoce permet une prise en charge efficace selon les mêmes modalités.

Summary

Dermatofibrosarcoma protuberans is a malignant and locally invasive tumor. It generally affects young adults. However, in rare but not exceptional cases, children can suffer from this disease. In the literature, there are only few studies on this pathology occurring in children. The main treatment is surgical excision with large margins. We studied in our series 15 cases of dermatofibrosarcoma in children between 1995 and 2008. The data were retrospectively collected. The aim of our study is to underline the best support for this pathology in children. The mean age at the moment of diagnosis was 13years old. None of our patients underwent surgery for dermatofibrosarcoma removal before. The diagnosis was confirmed by biopsy and immunology by FISH method. In 60% of the cases, the lesion, which is very polymorphic, was on the trunk. In three cases, a skin trauma was noted in the patient’s history. Surgical management consisted in tumor’s excision with a 3cm margin laterally associated with the removal of the first unaffected anatomical layer in depth. Reconstruction was adapted to the defect’s width, depth and topography. The mean follow-up was 7.8years. There were no relapses reported in a 3-year period at least. Complications were two scars dehiscences which needed skin graft for repair and a scarring alopecia reconstructed secondarily by a skin expansion protocol. No functional sequelae were reported in our series. As dermatofibrosarcoma in children presents the same clinical aspect and evolution as in adults, we treated it the same way. This diagnosis should not be ignored in children, and should be made early to give the patient the most effective support.
