Résumé

Les fractures à trois ou quatre fragments de l’humérus proximal sont le plus souvent traitées chirurgicalement. L’ostéosynthèse est la technique de choix chez le sujet jeune. Une réduction exacte des tubérosités est indispensable pour que, en cas de nécrose avasculaire mal tolérée, une prothèse humérale puisse être pratiquée dans de bonnes conditions, c’est-à-dire avec des tubérosités réduites. Le résultat de cette ostéosynthèse peut être décevant, l’articulation de l’épaule étant peu tolérante aux imperfections de réduction et de fixation. Le scanner préopératoire est indispensable. Il permet en effet d’analyser le déplacement et la comminution des fragments, de classer la fracture et d’évaluer la vitalité de la tête humérale, puis d’apprécier la qualité mécanique de l’os sous-jacent. La réduction de la fracture obéit à des principes que l’on retrouve dans les différentes techniques. La réduction de chaque fragment doit être étudiée séparément, en commençant par la réduction de la tête humérale sur la diaphyse puis en réduisant les tubérosités. Les deux complications les plus fréquentes sont l’ostéonécrose de la tête humérale, bien tolérée dans de nombreux cas, et le cal vicieux des tubérosités, par défaut de réduction et/ou de fixation, toujours mal toléré et difficile à corriger. Il faut donc choisir une technique d’ostéosynthèse permettant une réduction anatomique et une fixation solide des tubérosités, avec un risque limité de migration du matériel dans l’articulation. Cette conférence détaille les différentes techniques, avec leurs avantages et inconvénients, afin de permettre au chirurgien de choisir la mieux adaptée à la fracture qu’il doit opérer.

Abstract – Surgical treatment of three- and four-part proximal humeral fractures

Three- and four-part proximal humeral fractures are often treated surgically. ORIF is the gold standard in young patients; healing of the tuberosities after accurate reposition is indeed necessary to make it possible to perform secondary hemiarthroplasty under good conditions, if symptomatic avascular necrosis occurs. ORIF of proximal humeral fractures is challenging, as the shoulder is poorly tolerant to imperfection in reduction or fixation. Pre-operative CT-scan is mandatory. It makes it posible to assess the displacement and comminution of the fragments, to classify the fracture and to evaluate the mechanical properties of the bone and the risk of subsequent avascular necrosis of the humeral head. As regards the fracture reduction, some of the steps are common to all surgical techniques. Reduction of each fragment requires a specific planning, and the reduction of the humeral head on the shaft should come first. The way each fragment should be reduced is thoroughly described in this paper. The two main complications following ORIF are avascular necrosis of the humeral head, often well tolerated, and malunion of the tuberosities, due to improper reduction or fixation. Malunion of the tuberosities leads in all cases to a poor outcome and is difficult to correct. Consequently, the choice of the most appropriate surgical technique must take under consideration the need for anatomical reduction and strong fixation of the tuberosities, with limited risk of hardware migration into the joint. This paper gives a thorough description of the various techniques available, together with their advantages and disadvantages, in order to help surgeons in selecting the appropriate treatment for individual cases treated.
