Résumé

L’ostéonécrose spontanée du genou (OSG) a été décrite pour la première fois par Ahlback et al. en 1968 comme une zone de nécrose avasculaire comportant le condyle fémoral médian. L’OSG se présente le plus souvent chez des patients au-delà de l’âge de 55 ans, n’ayant aucun facteur de risque et de manière unilatérale dans un compartiment du genou. L’OSG peut être classifiée en quatre stades, basés sur des critères cliniques et radiologiques. Dans les stades III et IV, l’arthroplastie a un rôle prééminent dans le traitement de l’OSG. Elle semble être la seule option valable en cas d’ostéonécrose du genou où il y a eu un collapsus ou en cas d’échec d’autres options thérapeutiques. Même s’il y a peu de littérature concernant les résultats de l’arthroplastie en cas d’OSG, les rapports les plus récents montrent qu’une arthroplastie monocompartimentale dans l’OSG donne d’excellents résultats tant sur le plan clinique qu’en termes de survie.

Abstract

Spontaneous osteonecrosis of the knee (SONK) was first described by Ahlback et al. in 1968 as an area of avascular necrosis involving the medial femoral condyle. It tends to occur most often in patients older than 55 with no risk factors, unilaterally, and in one compartment of the knee. Most of the time, the medial femoral condyle is involved. SONK can be classified into four stages, based upon symptomatic and radiologic features. Considering stage III and IV arthroplasty plays a prominent role in the treatment of SONK. Joint replacement seems to be the only valuable option in case of osteonecrotic knee joints that have undergone collapse or where other treatments have failed. There is little literature reporting the results of arthroplasty for osteonecrosis of the knee. However, most recent literature reveals that UKA for SONK provides excellent clinical results and an excellent survival rate.
